摘 要:目的了解移居不同海拔不同时间高原移居者的超声心动图及变化特点,为高原移居者健康保健提供参考。方法对移居海拔5070 m 1个月、4300 m 4个月、3700 m 1年的移居者的超声心动图检测结果进行比较分析。超声心动图测量参数包括左室舒张末前后径(LVD)、左室收缩末前后径(LVS)、左房前后径(LA)、每搏心输出量(SV)、射血分数(EF)、右室前后径(RV)、右房左右径(RA)、肺动脉内径(PA)、肺动脉流速(PAV)及肺动脉收缩压(PAP)。结果移居5070 m组的LVD、LVS、SV显著低于4300 m组及3700 m组(P<0.05或P<0.01),其LA显著低于3700 m组(P<0.01),EF显著高于3700 m组(P<0.05)。5070 m组及3700 m组RA、RV均显著高于4300 m组(P<0.05或P<0.01),而5070 m组与3700 m组的RA、RV、PA、PAP均无统计学差异(P>0.05)。结论高原低氧环境对移居者心脏结构和功能均产生影响,海拔高度和移居时间是两个重要影响因素。移居海拔5070 m 1个月与移居3700 m 1年所造成的右心系统损伤程度相似。Objective To learn about the echocardiography and its changing characteristics of high altitude migrants residing at different altitudes at different time and provide reference for health care for the high altitude migrants.Methods The results of echocardiography of migrants at the altitude of 5070 m for one month,4300 m for four months and 3700 m for one year were compared and analyzed.The parameters measured in the echocardiography included LVD,LVS,LA,SV,EF,RV,RA,PA,PAV and PAP.Results The LVD,LVS and SV of the migrants in the group of 5070 m were significantly lower than those of migrants in the groups of 4300 m and 3700 m(P 〈0.05 or P 〈0.01).The LA of the migrants in the group of 5070 m was significantly lower than that of the migrants in the group of 3700 m(P 〈0.01) and the EF of that group significantly higher than that of the migrants in the group of 3700 m(P 〈0.05).The RA and RV of the migrants in the groups of 5070 m and 3700 m were significantly higher than those of the migrants in the group of 4300 m(P 〈0.05 or P 〈0.01),while there were no significant difference between the RA,RV,PA and PAP of the migrants in the groups of 5070 m and 3700 m(P 〈0.05).Conclusion The low oxygen environment at high altitude has impacts on the cardiac structure and function of migrants,with the altitude and the migration period as two major factors.Migrating to 5070 m for one month and to 3700 m for one year causes similar damage to the right heart system.
