环孢素A与吗替麦考酚酯治疗儿童难治性肾病综合征疗效对比研究

摘　　要：目的探讨环孢素A与吗替麦考酚酯治疗儿童难治性肾病综合征疗效。方法选取我院2012年3月至2015年3月的144例儿童难治性肾病综合征进行回顾性分析,随机分为Ⅰ组(肾上腺皮质激素抵抗型NS(SRNS)组)和Ⅱ组(肾上腺皮质激素依赖型NS(SDNS)组),Ⅰ组分为Ⅰa组(环孢素A+强的松)和Ⅰb组(吗替麦考酚酯+强的松)。Ⅱ组分为Ⅱa组(环孢素A+强的松)和Ⅱb组(吗替麦考酚酯+强的松)。研究治疗前及治疗后1月、3月、6月、12月治疗效果及不良反应。结果两组患者治疗3、6、12的24h尿蛋白定量、尿素氮、血清白蛋白、血肌酐、甘油三酯、总胆固醇明显优于治疗前和治疗后1个月,具有统计学意义(P<0.05);两组患者之间没有明显差异(P>0.05);两组患者的不良反应发生率之间没有明显差异(P>0.05)。结论采用环孢素A与吗替麦考酚酯治疗儿童难治性肾病综合征疗效具有相似的效果,均可以用于儿童难治性肾病综合征的治疗。OBJECTIVE To investigate the therapeutic effect of cyclosporin A and mycophenolate mofetil in the treatment of children with refractory nephrotic syndrome. METHODS 144 children with refractory nephrotic syndrome in our hospital from March 2012 to March 2015 were analyzed retrospectively and were divided for 2 groups：group Ⅰ（ adrenocortical steroid resistant NS（ SRNS） group） and group Ⅱ（ adrenocortical steroid dependent NS（ SDNS） group）. Children in group Ⅰ were divided into Ⅰa group（ cyclosporine a and prednisone） and Ⅰb group（ mycophenolate mofetil ＋ prednisone）. Ⅱ group was divided into group Ⅱa（ cyclosporine A ＋ and prednisone）group ⅡB（ mycophenolate mofetil ＋ prednisone）. Before treatment and after treatment about 1,3,6,12 months,the treatment effect and adverse reactions were studied. RESULTS After treatment aobut 3 months,6 months and 12 months,24h urine protein,urea nitrogen,serum albumin,serum creatinine,triglycerides,total cholesterol was significantly better than before treatment and 1 months after treatment,there was statistical significance（ p〈0. 05）. There was no significant difference between the two groups（ p〈0. 05）. There was no significant difference about the incidence of adverse reactions between the two groups（ p〈0. 05）. CONCLUSION The efficacy of cyclosporine A and mycophenolate mofetil in the treatment of children with refractory nephrotic syndrome is similar,they can be used in the treatment of children with refractory nephrotic syndrome.
