空腹血糖受损患者血尿酸与踝臂指数的相关性研究

摘　　要：目的观察血尿酸与空腹血糖受损（IFG）患者踝臂指数（ABI）的相关性。方法选取IFG患者300例，分别按照男性、女性血尿酸中位数值划分为高尿酸组和低尿酸组。测量人体学指标、血压，检测血尿酸、血糖、血脂、血肌酐等生化指标，放射免疫法检测空腹胰岛素，VistaAVS型周围血管诊断系统测量ABI。结果与低尿酸组比较，高尿酸组体患者ABI明显降低[两组男性分别为（1．02±0．14）和（O．83±0．13），女性分别为（1．06±0．12）和（0．89±0．13），P均〈0．01）；调整校正性别、年龄、吸烟史、饮酒史等混杂因素后，血尿酸与ABI呈明显负相关（r=-0．366，P〈0．01）；多因素逐步回归分析显示，血尿酸水平是ABI的独立影响因子（β=-0．426，P〈0．01）。结论血尿酸水平独立影响IFG患者的ABI，可能与IFG患者外周血管病变的发生密切相关。Objective To investigate the association between serum uric acid levels and ankle brachial index（ABI） in patients with impaired fasting glucose （IFG）. Methods 300 patients with IFG were recruited. According to the median of serum uric acid, males or females were divided into the high uric acid subgroup and low uric acid subgroup respectively. Anthropometry parameters as well as blood pressure, serum uric acid, blood glucose, blood lipids and creatinine were measured. Fasting insulin levels were determined with radioimmunoassay. Vista AVS peripheral vascular diagnosis system was used to assess ABI. Results When compared with low uric acid subgroup, ABI significantly decreased in high uric acid group[（1.02 ±0. 14） vs （0.83 ±0. 13） in males,（1.06±0.12） vs （0.89 ±0.13） in females, all P 〈 0.01 ]. After adjustment for age, sex, smoking and alcohol consumption, serum uric acid showed negative correlation with ABI （ r = - 0. 366, P 〈 0. 01 ）. Multiple stepwise regression analysis showed that serum uric acid was an independent impact factor of ABI （ β = - 0. 426, P 〈 0. 01 ）. Conclusion Serum uric acid levels are independently related with ABI in patients with IFG, which may contribute to the occurrence of peripheral arterial diseases in these population.
