摘  要:目的:探讨应用巴德3DMax补片行免钉合腹腔镜全腹膜外疝修补术(TEP)的临床应用价值。方法:回顾性分析126腹股沟疝患者行TEP手术的临床资料。结果:126例患者中,123例腹股沟疝均成功施行了TEP,24例术中出现腹膜撕裂,3例中转TAPP手术。其中单侧手术时间23~60min;平均39min,双侧46~120min,平均63min;术中出血5~15ml,平均10ml。手术次日均鼓励患者下床活动。术后住院2~4d,平均2.3d。术后13例出现伤口疼痛(10.3%),术后3~7d均可缓解,未出现术后慢性疼痛。术后出现腹股沟区血清肿18例(8.7%),均发生于横断疝囊的患者,经穿刺抽液和口服药物治疗后消失,其中8例(6.3%)患者出现下腹部暂时性感觉异常,尿潴留10例(7.9%),经再次留置导尿后缓解。未发生伤口感染和其他并发症。结论:应用巴德3DMax补片行免钉合的TEP手术具有手术创伤小,术后恢复快、并发症少、复发率低。Objective:To explore the Bard 3DMax patch-free stapling line clinical value of laparoscopic totally extra peritoneal hernia repair.Methods:The clinical data were retrospectively analyzed 126 patients undergoing TEP inguinal hernia surgery.Results:126cases of patients,123 cases of inguinal hernia were successfully underwent laparoscopic TEP,24 cases of intraoperative peritoneal tear occurred,3cases of transit TAPP surgery.Which unilateral operation time 23~60min,average 39 min,bilateral 46~120 min,average 63 min.Bleeding 5~15 ml,average10 ml.The next day surgery patients were encouraged to get out of bed.Postoperative hospital stay 2~4days,on average 2.3days.After 13 cases had wound pain(10.3%),after 3~7days can alleviate chronic pain after surgery does not appear.Postoperative seroma groin area in 18cases(8.7%),occurred in patients with transverse sac of fluid through the puncture disappeared after treatment with oral medications,including eight cases(6.3%)patients in the lower abdomen transient paresthesia,urinary retention in 10cases(7.9%),the ease again after catheterization.Wound infections and other complications.Were followed up for 6~24months after surgery.Conclusion:Applications Bard 3DMax patch row free surgical stapling of TEP has surgical trauma,faster recovery,fewer complications,recurrence rate,should be widely applied.
