甩肩疗法治疗肱骨外科颈骨折与钢板内固定术的临床对比研究

摘　　要：目的:比较甩肩疗法治疗肱骨外科颈骨折与钢板内固定术的疗效差异。方法:回顾性研究自2014年9月至2015年4月江苏省中医院骨伤科收治的41例肱骨外科颈骨折患者,其中17例行肱骨近端锁定钢板内固定(手术组),24例行甩肩疗法治疗(甩肩治疗组)。对比两组病例住院时间、骨折愈合时间、肩关节Constant评分。结果:两组年龄、性别、骨折分型无明显差异(P>0.05)。甩肩治疗组住院时间为(10.75±1.70)d明显短于手术组(14.65±2.37)d,骨折愈合时间为(13.96±1.20)周短于手术组(15.71±1.05)周,差异有统计学意义(P<0.01)。两组患者在术后6月肩关节Constant评分中疼痛评分,ADL,ROM,肌力及Constant总分差异无统计学意义(P>0.05)。结论:肱骨近端锁定钢板内固定术与甩肩疗法治疗NeerⅡ,Ⅲ型的肱骨外科颈骨折的疗效类似,但甩肩疗法住院时间及骨折愈合时间较短,操作简便,费用低廉,能尽可能减少对肩关节软组织的二次损伤,避免手术引起的创伤加重及血运破坏,从而有利于促进软组织功能的恢复以及骨折愈合。Objective：To compare clinical effect of lock proximal humerus plate and swing shoulder in treating humerus surgical neck fractures.Methods：Retrospective studied forty one patients with humerus surgical neck fractures from September 2014 to October 2015 in orthopedic department of Jiangsu province hospital of traditional Chinese medicine.Seventeen patients were treated with lock proximal humerus plate（surgery group）,and 24 patients were treated with swing shoulder method（swing shoulder group）.The length of stay,bone union time and Constant-Murley functional scoring were compared between two groups after treatment.Results：There were no significant difference in age,gender and fracture types between the two groups（P〉0.05）.The length of hospital stay in the swing shoulder group（13.96±1.20）days was shorter than the lock proximal humerus plate group（14.65±2.37）days.The bone union time in the operation group（15.71±1.05）weeks was longer than the swing shoulder group（13.96±1.20）weeks（P〈0.01）.There was no significant difference in pain score,ADL,ROM,myodynamia,and Constant scores between the two groups after treatment（P〉0.05）.Conclusion：The clinical efficacy of lock proximal humerus plate in treatment of Neer type Ⅱ and typeⅢ of humerus surgical neck fractures is similar to swing shoulder.However,the time of hospitalization and fracture healing time is shorter,the operation is simpler and the cost is lower in swing shoulder group.It can reduce the second injury of parenchyma around the shoulder joint and avoid the injury caused by the operation.It can promote the recovery of soft tissues and fractures.
