注射用膦甲酸钠联合加巴喷丁胶囊治疗老年带状疱疹的疗效观察

摘　　要：目的探讨注射用膦甲酸钠联合加巴喷丁胶囊治疗老年带状疱疹的临床疗效。方法:选择2012年1月—2015年1月收治的178例患者,按随机数字表法分成观察组(93例)和对照组(85例)。2组患者均静脉注射甲钴胺注射液0.5 mg/d,并给予脉冲激光照射患处,1日1次。在上述治疗的基础上,观察组患者给予注射用膦甲酸钠联合加巴喷丁胶囊,对照组给予阿昔洛韦注射液,治疗时间均为10 d,观察2组患者的疗效,睡眠状况、对生活的影响程度,止疱、止痛及结痂时间,不良反应及带状疱疹后遗神经痛发生情况。结果:观察组患者的总有效率为92.47%(86/93),明显高于对照组的74.12%(63/85),差异有统计学意义(P<0.05);2组患者睡眠状况及对生活质量的影响程度均较治疗前有明显改善,且观察组患者的改善情况明显优于对照组,差异有统计学意义(P<0.05);观察组患者止疱时间、止痛时间、结痂时间均明显短于对照组,差异有统计意义学(P<0.05);观察组患者后遗神经痛发生率为5.38%(5/93),对照组为18.82%(16/85),2组的差异有统计意义学(P<0.05);2组患者均未见明显不良反应。结论:注射用膦甲酸钠联合加巴喷丁胶囊治疗老年带状疱疹疗效较好,安全性较高,值得临床推广。OBJECTIVE： To investigate the clinical eficacy of foscarnet sodium for injection combined with Gabapentin capsules in treatment of elderly patients with herpes zoster. METHODS： 178 cases admitted from Jan. 2012 to Jan. 2015 were selected to be divided into observation group（ 93 cases） and control group（ 85 cases） via the random number table. Two group of patients were treated with 0. 5 mg / d intravenous methycobal,pulse laser on the affected area,once a day. Based on the conventional treatment,the observation group were given foscarnet sodium for injection combined with Gabapentin capsules,while the control group received acyclovir for injection. All the patients were treated for 10 days. The efficacy,sleep status,effects on life quality,time of response,analgesia and scab form,incidence of adverse reactions and postherpetic neuralgia were observed between two group of patients. RESULTS： The total effective rate of observation group was 92. 47%（ 86 /93）,significantly higher than that of control group [74. 12%（ 63 /85） ],with statistically significant difference（P〈0. 05）. The sleep quality and effects on life quality had been significantly improved compared with before treatment,and the improvement of observation group was better than that of control group,with statistically significant difference（P〈0. 05）. Time of response,analgesia and scab form in observation group was significantly lower than those in control group,with statistically significant difference（P〈0. 05）. The incidence of postherpetic neuralgia of observation group was 5. 38%（ 5 /93）,and control group was 18. 82%（ 16 /85）,the difference was statistically significant（P〈0. 05）. There were no significant adverse reactions in two group of patients. CONCLUSIONS： The efficacy of foscarnet sodium for injection combined with Gabapentin capsules in treatment of elderly patients with herpes zoster is effective,with high safety,which is worthy of clinical application and promotion.
