北京医院住院患者糖皮质激素类药应用分析

摘　　要：目的：了解北京医院（以下简称＂我院＂）住院患者糖皮质激素类药的使用情况。方法：利用医院信息系统,调查2008—2014年我院住院患者糖皮质激素类药（口服剂型和注射剂型）的使用情况,包括使用人次、销售金额、用药频度（defined daily dose system,DDDs）及不同科室的用药倾向等。结果：我院使用糖皮质激素类药的患者中,女性略多于男性;患者年龄段集中在50~〈80岁。2008—2014年我院糖皮质激素类药的销售金额、使用人次呈上升趋势;销售金额排序居前3位的药品分别为甲泼尼龙琥珀酸钠注射液、甲泼尼龙片和复方倍他米松注射液;DDDs排序较靠前的药品有地塞米松注射液、醋酸泼尼松片以及地塞米松片。我院免疫内科、神经内科、肾内科、呼吸内科、血液内科等主要收治免疫系统疾病患者的内科系统科室对甲泼尼龙及泼尼松片的选择倾向性大,其销售金额也高;而外科系统科室更倾向于选择复方倍他米松注射液、地塞米松磷酸钠注射液。结论：我院糖皮质激素类药的销售金额排序与其DDDs排序并不一致,不同科室对药品的品种选择有所差别。OBJECTIVE： To investigate the application status of glucocorticoid drugs in inpatients of Beijing Hospital（ hereinafter referred to as ＂our hospital＂）. METHODS： HIS of our hospital was adopted,the application status of glucocorticoid drugs（ oral liquid and injection type） in inpatients of our hospital during 2008-2014 were investigated in terms of cases,consumption sum,defined daily dose system（ DDDs） and tendency of drug application in different departments,etc. RESULTS： Of all the patients with glucocorticoid drugs,females were more than men.The age of patients were mainly concentrated in 50- 80 years old. The consumption sum,cases of glucocorticoid drugs in our hospital during 2008-2014 were in an increase tendency. The top three drugs ranked by consumption sum were respectively methylprednisolone sodium succinate for injection, Methylprednisolone tablets and compound betamethasone injection. Drugs that took the lead ranked by DDDs were respectively dexamethasone acetate injection and Prednisone tablets and Dexamethasone tablets. The amounts of consumption sum and DDDs of methylprednisolone and Prednisone tablets were large in the rheumatology department, neurological department, renal department,respiration department and blood department. But the amounts of consumption sum and DDDs of compound betamethasone injection, dexamethasone sodium phosphate injection were large in surgical department.CONCLUSIONS： The ranking of consumption sum and DDDs of glucocorticoid drugs was not uniform,and the selection of drugs were also different in different departments.
