超声引导下异位妊娠胚囊内注入无水酒精与MTX的疗效比较

摘　　要：目的比较介入性超声应用无水酒精与MTX（甲氨蝶呤）治疗异位妊娠的疗效。方法选取2010年10月～2014年2月在我院行超声引导下异位妊娠胚囊内注入无水酒精44例（酒精组）和注入MTX49例（MTX组）,比较两组治疗的有效率、7 d内包块血流消失率、7 d内血β-HCG下降率、副反应发生率、住院时间、包块消失时间。结果酒精组有效率略高于MTX组,差异无统计学意义（P〉0.05）。酒精组7 d内包块血流消失率、血β-HCG下降率明显高于MTX组,患者住院时间、包块消失时间明显短于MTX组,副反应发生率明显少于MTX组,差异有统计学意义（P〈0.05）。结论超声引导下异位妊娠胚囊内注入无水酒精与注入MTX比较,前者有效率高于后者,前者起效快、患者住院时间较短及副反应少,是一种较好的局部治疗异位妊娠的方法。Objective To compare the efficacy between anhydrous alcohol and MTX under the interventional ultrasound in ectopic pregnancy.Methods The patients under the guidance of ultrasound in ectopic pregnancy from October 2010 to February 2014 were enrolled,44 cases were injected with anhydrous alcohol,49 cases were injected with MTX.The rate of efficacy,the rate of disappearance of blood supply of the mass in the seven days,the rate of decrease of β-HCG in the peripheral blood in the seven days,the rate of side effects,the length of hospital stay,and the time of disappearance of mass between the two groups were compared.Results The rate of efficacy was slightly higher in the anhydrous alcohol group,but there was no statistical difference compared with the MTX group（P〈0.05）;the rate of disappearance of blood supply of the mass in the seven days and the rate of decrease of β-HCG in the peripheral blood in the seven days were apparently higher,the length of hospital stay and the time of disappearance of mass were apparently shorter,and the rate of side effects was apparently lower in the alcohol group compared with the MTX group,there were statistical differences（P〈0.05）.Conclusion Embryo sac injection of anhydrous alcohol is compared with embryo sac injection of MTX under the guidance of ultrasound in ectopic pregnancy,the rate of efficacy of embryo sac injection of anhydrous alcohol is higher,and there are faster acting,shorter hospital stay and fewer side effects,so it is a better way to local treat ectopic pregnancy.
