锁定钢板与双切口双钢板内固定治疗复杂性胫骨平台骨折的疗效比较

摘　　要：目的 比较锁定钢板与双切口双钢板内固定治疗复杂性胫骨平台骨折的临床疗效。方法 将我院2011年2月～2014年3月收治的172例复杂性胫骨平台骨折患者按随机数字表法则分为锁定钢板组与双切口双钢板组各86例,锁定钢板组采用锁定钢板内固定治疗,双切口双钢板组采用双切口双钢板内固定治疗。观察比较两组患者的手术时间、术中出血量、住院时间、骨折愈合时间及完全负重时间等指标。结果 锁定钢板组的手术时间较双切口双钢板组显著缩短（P〈0.05）,术中出血量及住院时间组间比较差异无统计学意义（P〉0.05）;锁定钢板组的骨折愈合时间及完全负重时间均显著短于双切口双钢板组（P〈0.05）;锁定钢板组与双切口双钢板组的术后并发症发生率分别为9.3%与11.6%,组间比较差异无统计学意义（P〉0.05）;术后1年,锁定钢板组与双切口双钢板组的Rasmussen评分优良率分别为72.1%与70.9%,组间比较差异无统计学意义（P〉0.05）。结论 两种术式对复杂性胫骨平台骨折的治疗在有效性与安全性两方面效果相当,但锁定钢板内固定治疗在手术时间、骨折愈合时间以及完全负重时间方面较双切口双钢板内固定治疗更具优势。Objective To compare the clinical efficacy of locking steel plate and double incision double steel plate internal fixation in the treatment of complex tibial plateau fracture. Methods A total of 172 patients with complex tibial plateau fracture treated from February 2011 to March 2014 were divided into the locking steel plate group and the double incision double steel plate group according to the random number table method, with 86 patients in each group. The locking steel plate group was given locking plate internal treatment and the double incision double steel plate group was given double incision double steel plate internal fixation. The operation time, amount of bleeding during operation, hospital stay, fracture healing time and full-weight-bearing time were observed and compared between the two groups. Results The locking steel plate group was significantly shorter than the double incision double steel plate group in the surgical time （P〈0.05） and the two groups were not statistically different in the intraoperative bleeding amount and length of hospital stay （P〉0.05）. The locking steel plate group was significantly shorter than the double incision double steel plate group in the fracture healing time and full weight-bearing （P〈0.05）. The incidences of postoperative complications of the locking steel plate group and the double incision double steel plate group were 9.3% and 11.6% respectively, without statistically significant difference（P〉0.05）. One year after surgery, the excellent and good rates of Rasmussen scores of the locking steel plate group and the double incision double steel plate group were 72.1% and 70.9% respectively, without statistically significant difference （P〉0.05）.
