糖皮质激素联合加巴喷丁对带状疱疹疼痛及后遗神经痛的疗效观察

摘　　要：目的 观察糖皮质激素联合加巴喷丁对带状疱疹（herpes zoster,HZ）疼痛治疗疗效及对带状疱疹后遗神经痛（postherpetic neuralgia,PHN）形成的影响。方法 选择2014年8月～2015年8月我院皮肤科带状疱疹120例随机分为观察组和对照组各60例：观察组口服强的松片联合加巴喷丁胶囊,对照组口服双氯芬酸钠缓释胶囊镇痛治疗,均以疼痛消失停止用药;两组均给予更昔洛韦针抗病毒治疗,10 d左右。并于治疗前、治疗第2周、第4周以及第12周对形成PHN患者进行VAS评分、生活质量评分。结果 带状疱疹治疗的生活质量、VAS评分比较：治疗第2、4周评分下降,以观察组下降明显,差异有统计学意义（P〈0.05或P〈0.01）;PHN发生率：观察组和对照组发生率分别为16.7%,35.0%,差异有统计学意义（χ2=5.263,P=0.022）;12周后两组PHN患者生活质量、VAS评分比较差异有统计学意义（P〈0.05）。结论 糖皮质激素联合加巴喷丁对带状疱疹疼痛疗效显著,并可降低PHN的形成,加巴喷丁可有效提高PHN生活质量。Objective To observe the effect of glucocorticoid combined gabapentin capsules in treatment of herpes zoster（HZ） pain and postherpetic neuralgia（PHN）. Methods A total of 120 cases patiens with HZ from August 2014 to August 2015 were randomly divided into observation group and control group, which was 60 cases in each. When both groups were treated with ganciclovir injection in course of about 10 days, the observation group was orally given prednisone tablets combined with gabapentin capsules, and control group was orally given diclofenac tablets, and both stopping the treatment when pain disappeared. The visual analogue scale（VAS） and quality of life score of PHN patients were observed before treatment and 2, 4 weeks and 12 weeks after treatment. Results The quality of life and VAS score of HZ showed that the observation group decreased more significantly than that in control group in 2, 4 weeks（P〈 0.05 or P〈0.01）. The incidence of PHN in the observation and control group separately was 16.7%, 35.0%, the differ ence was statistically significant（X2=5.263, P=0.022）, quality of life and VAS score of patients with PHN after 12 weeks were significant statistically between two group（P〈0.05）. Conclusion Glucocorticoid combined with gabapentin has significant therapeutic effect on herpes zoster pain, and can reduce the formation of PHN, gabapentin can effectively improve the life quality of PHN patients.
