联合检测ANA、抗SSA和抗SSB对原发性干燥综合征诊断的临床意义

摘　　要：目的调查浙江地区原发性干燥综合征患者的抗核抗体及抗核抗体谱阳性分布特点及临床意义。方法采用间接免疫荧光法和欧蒙线性免疫印迹法,回顾性分析2012年6月~2015年6月我院就诊的162例临床确诊原发性干燥综合征患者血清中抗核抗体及其抗核抗体检测谱的结果。结果 162例研究对象中抗核抗体阳性149例(占92.0%)。男7例,女155例。抗SSA阳性118例(占72.8%);抗SSB阳性72例(占44.4%);抗Ro52阳性119例(占73.4%);三者阳性检出率显著高于对照组,差异有统计学意义(P<0.05)。其他自身抗体检出率均较低。ANA分别与抗SSA、抗SSB、抗Ro52联合检测的阳性率为87.6%、65.9%、83.2%。联合检测抗SSA、抗SSB具有相对较好的特异性为57%。结论 ANA阳性患者,联合抗SSA、抗SSB判断有助于提高原发性干燥综合征诊断的特异性。Objective To study the distribution characteristics and clinical value of antinuclear antibodies（ANA） and ANAs spectrum of the primary sj^gren＇s syndrome （pSS） patients in Zhejiang province. Methods Indirect fluorescence （IIF） and Euroline ANA Profile methods were used to detect the 162 serum specimens of pSS patients in Zhejiang Provincial People＇s Hospital from June 2012 to June 2015, then analyzed the data. Results Among the 162 pSS patients,which including 7males and 155 females. The ANA of the 149 pSS patients were positive （92.0%）. The positive rate of SSA, SSB and Ro52 antibodies were 118（72.8%）, 72（44.4%） and 119 （73.4%） respectively. The three kinds of antibodies were significantly higher than control group （P〈0.05）. The positive rate of ANA combine detection of SSA, SSB and Ro52 were 87.6%.65.9% and 83.2% respectively. The Specificity of combined detection of SSA and SSB was 57%. Conclusion In the ANA positive specimens, combined detection of SSA and SSB will improve the specificity of the pSS patients.
