介入手术治疗心血管疾病后迷走神经异常反射的影响因素

摘　　要：目的 分析介入手术治疗心血管疾病后迷走神经异常反射的影响因素.方法 回顾性分析2009年-2013年于武汉市中心医院接受介入手术治疗的109例心血管疾病患者,男性64例,女性45例,年龄37-68（48.57±8.32）岁.根据是否出现迷走神经异常反射分为2组对照组67例,异常反射组42例.对两组临床资料进行单因素分析和多元Logistic回归分析,分析迷走神经异常反射的相关因素.结果 两组患者年龄、性别构成比例、体质指数、合并高血压病、糖尿病、高酯血症比例比较,差异均无统计学意义（P均〉0.05）.与对照组比较,异常反射组精神状态紧张、术前禁食时间〉4 h、经股动脉穿刺及术后立即拔管比例均升高,差异有统计学意义（P均〈0.05）.多元Logistic回归分析,可知介入手术治疗心血管疾病后迷走神经异常反射的危险因素有患者的精神状态紧张、术前禁食时间〉4 h、经股动脉穿刺及术后立即拔管（P均〈0.05）.结论 介入手术治疗心血管疾病后迷走神经异常反射的危险因素包括患者的精神状态紧张、术前禁食时间〉4 h、经股动脉穿刺及术后立即拔管,临床医生应予以重视.Objective To analyze the influencing factors of abnormal vagal reflex after treatment ofcardiovascular diseases with interventional operation. Methods The patients （n=109, male 64, female 45, agedfrom 37 to 68 and average age=48.57±8.32） were chosen from 2009 to 2013. All patients were divided, accordingto whether or not with abnormal vagal reflex, into control group （n=67） and abnormal group （n=42）. The clinical datawere given single-factor analysis and Logistic regression analysis, and relevant factors of abnormal vagal reflex wereanalyzed in 2 groups. Results The difference in age, sex proportions, body mass index （BMI）, and percentages ofcases complicated by hypertension, diabetes and hyperlipidemia had no statistical significance （all P〉0.05）. Thepercentages of patients with mental stress, pre-operation fasting time〉4 h, transfemoral puncture and immediatepostoperation extubation increased in abnormal group compared with control group （all P〈0.05）. The results of Logisticregression analysis showed that the risk factors of abnormal vagal reflex included mental stress, pre-operation fastingtime〉4 h, transfemoral puncture and immediate postoperation extubation after treatment of cardiovascular diseaseswith interventional operation （all P〈0.05）. Conclusion The risk factors of abnormal vagal reflex include mental stress,pre-operation fasting time〉4 h, transfemoral puncture and immediate postoperation extubation after treatment ofcardiovascular diseases with interventional operation, and clinical physicians should attach importance to them.
