摘　　要：目的：通过对2015年医院外科手术病人抗菌药物医嘱点评结果汇总及不合理医嘱的分析，找出不合理主要原因，为手术病人抗菌药物的合理使用提供指导。方法：抽取我院2015年度手术病人抗菌药物医嘱3150份，对抗菌药物的合理率、不合理医嘱类型的分布情况进行统计，并对各类不合理医嘱进行点评分析。结果：在抽出的3150份病例中，合理病例2968份，占整体的94.2%。不合理医嘱中手术预防用抗生素选择不适宜、用法用量不适宜占主要因素。结论：医院2015年度抗菌药物用药合理率得到了稳步的发展，临床药师在此过程中发挥着重要作用，但仍需加强医生对制度规范的掌握，增强医院多部门协调合作，让问题充分反馈，进一步提高医生临床用药水平。Objective：Through the summary of the results of antibacterial prescriptions and analysis of irrational medical advice of surgical patients in 2015, finds out the main unreasonable causes, in order to provide guidelines for the rational use of antibacterials in surgical patients. Methods：3150 cases of antibacterial prescriptions for surgical patients in 2015 were extracted, counted the reasonable rate of antibacterials and the distribution of the types of irrational medical advices, and commented and analyzed the various types of irrational medical advices. Results：In 3150 cases, 2968 cases were reasonable prescriptions, accounting for 94.2%. In irrational medical advices, the inadequacy of antibacterials selection for surgical pro-phylaxis and the usage and dosage were the main factors. Conclusion：The reasonable rate of antibacterials medication in 2015 has been steadily de-veloped, clinical pharmacists have played an important role in this process, but it still need to strengthen the doctors＆#39; master level of institutional norms, enhance the coordination among departments in hospital, which can make problems fully feedback, and further improve the medication level of doctors.
