摘 要:目的探讨分步延期手术治疗高能量Pilon骨折的应用价值。方法选取106例高能量Pilon骨折为研究对象,随机分为观察组和对照组,每组各53例。观察组采用分步延期手术治疗,对照组采用传统早期切开复位内固定治疗,比较两组患者的手术情况、骨折愈合情况及并发症的发生率。结果观察组手术时间、术中出血量、骨折影像学愈合时间、平均负重下地时间、手术后并发症(创伤性关节炎、骨不连和踝关节僵硬)均明显低于对照组;观察组术后3个月、6个月和12个月Baird踝关节评分明显高于对照组(P<0.05)。结论分步延期手术治疗高能量Pilon骨折患者可减轻手术创伤、促进骨折愈合、降低并发症并改善踝关节功能。Objective: To investigate the application value of fractional delayed surgery for treatment of patients with high energy Pilon fracture. Methods: 106 patients with high energy Pilon fracture were included and divided into the obser- vation group and the control group with 53 patients in each group. Patients in the observation group were treated with frac- tional delayed surgery, while patients in the control group with traditional surgery of early open reduction and internal fixa- tion. Incidence of surgery, fracture healing and complications were compared between the two groups. Results : Observa- tional indexes including surgery time , the amount of surgery bleeding, radiographic healing time of fracture, average time of heavy burden, and incidence of complications after surgery such as traumatic arthritis, bone nonunion and ankle joint stiffness were significantly lower than those in the control group. Baird ankle joint scoring of 3 months, 6 months and 12 months after surgery were significantly higher than those in control group ( P 〈 0. 05 ). Conclusion: These results showed that fractional delayed surgery for patients with high energy Pilon fracture may reduce surgical wounds, promote fracture healing, decrease incidence of complications and improve function of ankle joint.
