内镜下镜下OTSC在胃全层切除术中的闭合作用评价

摘　　要：【目的】对比OTSC及金属夹在内镜下胃全层切除术后闭合作用,评估OTSC闭合效果。【方法】回顾性研究2012年6月至2015年6月因胃固有肌层肿物于我内镜中心行内镜下全层切除的54例患者,根据闭合方式分成金属夹闭合组(17例)及OTSC闭合组(37例),对比两组的闭合时间、住院周期、费用及术后并发症。【结果】全层切除手术成功率为100%,闭合成功均为100%。OTSC可明显缩短闭合时间(P<0.05),缩短住院周期(P<0.05),且术后并发症发生率相对较少,但费用相比金属夹较高(P<0.05)。【结论】应用OTSC闭合内镜下胃全层切除缺损创面安全,有效,快捷,但费用相对金属夹较高。【Objective】To contrast the closure efficacy of OTSC and metallic clip after endoscopic gastric full-thickness resection, and evaluate the effect of OTSC closure.【Methods】The cases with gastric submucosal tumors originating from the muscularis propria layer undergoing endoscopic full-thickness resection were selected in our endoscopy center from June 2012 to June 2015. The data were retrospectively studied. According to the closure mode, they were divided into metallic-clip group and OTSC group. The duration of closure, hospitalization period, medical expense and postoperative complications in both groups were contrasted.【Results】A total of54 patients were recruited into this study, 17 patients in metallic-clip group and 37 cases in OTSC group. The success rate of fullthickness resection was 100%. Moreover, the success rate of closure was also 100%. Compared with metallic-clip group, the duration of closure and hospitalization period in OTSC group were shorter（P〈0.05）, and the incidence of postoperative complications was lower, but the medical expense in OTSC group was higher（P〈0.05）.【Conclusion】The application of OTSC for closing the gastric defects in endoscopic full-thickness resection is safe, effective, fast and expensive.
