摘 要:目的分析婴幼儿肺炎患儿血清25-羟维生素D[25-(OH)D3]及CD4+/CD8+水平,探讨维生素D(Vit D)与婴幼儿肺炎以及免疫的关系。方法收集2013年2月至2015年6月在洛阳市第六人民医院儿科收治的120例婴幼儿肺炎患者作为病例组,进一步将病例组随机分为常规治疗组60例和Vit D补充组60例,Vit D补充组在常规治疗的基础上补充Vit D。同期采用随机数字法收集在本院健康体检的120例健康婴幼儿作为对照组。治疗前检测血清25-(OH)D3、CD4+、CD8+、CD4+/CD8+水平并进行组间比较。治疗3个月后,病例组复查血清25-(OH)D3、CD4+、CD8+、CD4+/CD8+水平,在不同治疗方案两组儿童中比较。结果治疗前病例组血清25-(OH)D3及CD4/CD8水平显著低于健康对照组,差异有统计学意义(t=3.36~31.6,P〈0.05),而病例组常中常规治疗组与补充VITD组间无统计学差异(t=0.31~1.52,P〉0.05)。补充Vit D治疗3个月后,Vit D补充组25-(OH)D3及CD4/CD8水平显著高于常规治疗组(t=3.02~33.6,P〈0.05)。结论婴幼儿肺炎患儿血清25-(OH)D3及CD4/CD8水平低于健康儿童。低水平25-(OH)D3与婴幼儿免疫功能和肺炎有关,提示对肺炎患儿应重视Vit D的补充。Objective To analyze the levels of serum 25 hydroxy-vitamin D [25- (OH) D3 ] and CD4 +/CD8 + in infants with pneumonia, and to explore the relationship between Vitamin D (VitD) and pneumonia as well as immunization in infants. Methods A total of 120 infants with pneumonia were selected in case group. All these infants received treatments at the department of pediatrics in the Sixth People' s Hospital of Luoyang from February 2013 to June 2015. Moreover, those patients were randomly divided into conservation therapy group and VitD supplemented group, 60 cases for each group. Infants in the VitD supplemented group received VitD on the basis of conventional therapy. Over the same period, 120 healthy infants who underwent physical examinations at the same hospital were randomly selected in control group. Levels of25-(OH)D3, CIM + , CD8 + , CD4 +/CD8 + in serum were detected and compared among three groups before treatment. After 3 months of treatment, levels of25-(OH)D3 , CD4 + , CD8 + , CD4 +/CD8 + in serum were re-examined in the case group. Results were compared between groups of different therapies. Results Before treatment, levels of serum 25-(OH) D3 and CD4/CD8 in infants of case group were significantly lower than those of the control group. The difference were statistical significance (t=3.36 -31. 6, P 〈 0. 05), while there was no statistically significant difference between conservation therapy group and VitD supplemented group ( t = 0. 31 - 1.52, P 〉 0.05 ). After 3 months of VitD supplemented treatment, levels of 25-(OH) D3 and CD4/CD8 in VitD supplement group were significantly higher than those of conservation therapy group ( t = 3.02 - 33.6, P 〈 0.05 ). Conclusion Infants with pneumonia have lower levels of 25-(OH) D3 and CIM +/CD8 + in serum than healthy children. Low levels of 25-(OH) D3 are related to immune function and pneumonia in infants, indicating that attention should be paid to the supplement of VitD children in infan
