摘  要:目的探讨体表巨大静脉畸形的优化治疗方案.方法颜面部静脉畸形患者主要接受平阳霉素瘤内注射或无水乙醇瘤内注射治疗;躯干、四肢静脉畸形患者以瘤内铜针埋置、置铜针术前或(和)术后平阳霉素瘤内注射、术后单发或小范围病灶手术切除、穿戴弹力衣等手段联合治疗.结果本组共57例患者.颜面部静脉畸形27例,24例接受3~5次平阳霉素瘤内注射后均有效;3例接受2、3次无水乙醇注射治疗后有效.躯干、四肢静脉畸形30例,5例行平阳霉素+手术切除治疗,4例治愈,1例显效;25例行铜针治疗+平阳霉素治疗,13例治愈,8例显效,4例有效.结论体表巨大静脉畸形的治疗方案应以硬化剂注射、铜针、手术等多种手段并用,针对不同患者选择个体化的组合方案,方能取得满意的疗效.Objective To investigate the optimization of treatment regimen selection for superficial giant venous malformations.Methods Patients with facial malformations received major inner intratumoral injection of pingyangmycin or ethyl alcohol;patients with trunk and limb malformations received intratumoral embedment of copper needles.Pingyangmycin intratumoral injection was also performed before and/or after the embedment.Postoperative single or small tumors were treated with combined treatment,such as surgical ablation or compression clothing.Results Of 57 cases,27 cases with facial malformations,24 cases were resolved after 3 to 5 times pingyangmycin injection,while 3 cases were resolved after 2 or 3 times dehydrated alcohol injection.Of 30 cases with trunk and limb malformations,5 cases received pingyangmycin injection and surgical resection,4 cases were cured and 1 case was excellent;25 cases received embedment of copper needles and pingyangmycin injection,13 cases were cured,8 cases were excellent,4 cases resolved.Conclusion The treatment programs for superficial giant venous malformations should be based on varied methods,such as sclerotherapy,copper needles,surgery and other means.Individual combined treatments for different patients can achieve satisfactory results.
