RPH与PPH治疗老年重度痔的疗效对比研究

摘　　要：为比较RPH与PPH治疗老年重度痔的手术效果，对Ⅳ期环状内痔老年患者（〉65岁）75例采用2种手术方式治疗，其中PPH组39例，RPH组36例，比较2组患者平均手术时间、住院时间、术中血压波动、术后并发症及术后复发率等指标。结果显示，术后大出血、住院时间、并发尿潴留、术中血压波动发生率RPH组优于PPH组（P〈0．05），患者平均手术时间、术后疼痛、创口感染、肛门狭窄、肺部感染及复发率等2组无显著性差异（P〉0．05）。结果表明，PPH和RPH均是治疗老年重度痔的有效方法，RPH有操作更简单、并发症更少的优点。This study was to compare the surgical efficacies of Ruiyun procedure for hemorrhoids（RPH） and PPH for senile severe hemorrhoids.Among the 75 patients （〉65 yr.,age） with IV stage circumferen- tial internal hemorrhoids 39 received PPH（as PPH group）,36 received RPH（as RPH group）;Both group＇s following indexes were observed and compared, including average hospitalization time, operative time, and intraoperative blood pressure changes, postoperative complication, as well postoperative recurrence rate, etc.As results,in postoperative massive bleeding and hospitalization time, complicated uroschesis,and intr- aoperative blood pressure change,RPH group was all superior to PPH group（ P〈0.05）,but in average operative time, and postoperative pain, wound infection, anal stricture, lung infection as well recurrence rate there was no significant difference between two groups（ P 〉0.05）.Results show that though above two procedure for senile severe hemorrhoids all are effective surgical method,but RPH is perfect for more simple manipulation, more less complication.
