摘  要:目的探讨睡眠呼吸障碍对儿童睡眠结构及图片学习记忆能力的影响。方法选择原发性鼾症(PS)患儿33例(PS组)、阻塞性睡眠呼吸暂停综合征(OSAS)43例(OSAS组),及同期健康体检儿童29例(对照组)。三组均行多导睡眠监测及图片学习记忆能力测试。结果三组总睡眠时间(TST)、睡眠效率、睡眠潜伏期、入睡后清醒时间、快眼动睡眠占TST比例比较差异均无统计学意义(P均>0.05)。与对照组比较,OSAS组非快眼动睡眠3期占TST比例(N3%)、SpO_2、最低血氧饱和度(MinSpO_2)降低,非快眼动睡眠1期占TST比例(N1%)、呼吸暂停/低通气指数(AHI)、阻塞性呼吸暂停指数(OAI)、低通气指数(HI)、氧减指数(ODI)增加(P均<0.01);PS组N3%减少,非快眼动睡眠2期占TST比例(N2%)、AHI、OAI、HI增加(P均<0.01)。OSAS组N1%、AHI、OAI、MAI、HI、ODI高于PS组,N2%、Min SpO_2、SpO_2低于PS组(P均<0.01)。三组图片学习记忆能力测试结果比较差异无统计学意义(P>0.05)。结论 PS、OSAS患儿均存在睡眠结构紊乱;疾病严重程度及睡眠结构紊乱对患儿的图片学习记忆能力无明显影响。图片学习记忆能力测试不能全面反映睡眠呼吸障碍对儿童学习记忆能力的影响。Objective To investigate the influences of sleep disordered breathing on sleep structure and pictorial-based memory of children. Methods Thirty-three children with primary snoring( PS group),43 with obstructive sleep apnea syndrome( OSAS group) and 29 healthy children( control group) were chosen. They all underwent a sleep study( overnight polysomnogram) and completed a pictorial-based memory task. Results There was no significant difference in total sleep time( TST),sleep efficiency( SE),sleep latency( SO),wake after sleep onset( WASO) and non-rapid eye movement time / TST( REM%) among the three groups( all P〉 0. 05). Compared with the control group,OSAS group had lower N3%,SpO_2,Min SpO_2 and higher N1%,AHI,OAI,HI,ODI( all P〈 0. 01). PS group had lower N3% and higher N2%,AHI,OAI and HI( all P〈 0. 01). OSAS group had higher N1%,AHI,OAI,HI,ODI and lower N2%,SpO_2 Min SpO_2 as compared with that of the PS group( all P〈 0. 01). There was no significant difference in the score of pictorial-based memory task among the three groups( P〉 0. 05). Conclusions Both PS and OSAS children have abnormal sleep structure,but the severity of disease and abnormal sleep structure has no effect on the pictorial-based memory. We couldn' t demonstrate that sleep disordered breathing can affect children's ability of learning picture from the pictorial-based memory task in our study.
