三子止咳胶囊对COPD稳定期患者肺功能的改善作用及机制

摘　　要：目的观察三子止咳胶囊对慢性阻塞性肺疾病(COPD)稳定期患者肺功能的改善作用,探讨其可能的作用机制。方法将68例COPD稳定期患者随机分为观察组33例、对照组35例。观察组口服三子止咳胶囊,对照组口服安慰剂,均2粒/次,3次/d,治疗6个月。分别于治疗前及治疗6个月,应用肺功能检测仪检测第1秒用力呼气量(FEV1)、用力肺活量(FVC)、FEV1占预计值百分比(FEV1%pred),测定患者血清脂联素(APN)、白细胞介素17(IL-17)、基质金属蛋白酶9(MMP-9)、基质金属蛋白酶抑制剂1(TIMP-1)水平。结果与治疗前比较,治疗6个月对照组FVC、FEV_1、FEV1%pred及血清APN、IL-17、MMP-9、TIMP-1水平比较差异无统计学意义(P均>0.05),观察组以上指标均改善(P均<0.05),治疗6个月两组以上指标比较差异有统计学意(P均<0.05)。结论三子止咳胶囊对COPD稳定期患者的肺功能有改善作用,该作用可能与降低血清APN、IL-17、MMP-9、TIMP-1水平有关。Objective To observe the improvement effect of Sanzi Zhike capsule on the lung function in patients with stable COPD,and to explore the possible mechanism of the action. Methods Sixty-eight patients with COPD in stable stage were randomly divided into 2 groups： observation group（ treated with Sanzi Zhike capsule,2 capsules each time,three times per day for 6 months,n = 33） and control group（ treated with placebo,the same dosage,n = 35）. The lung function detector was used to detect first second forced expiratory volume（ FEV1） and forced vital capacity（ FVC）,forced expiratory volume in 1 second / expected value %（ FEV1% pred）,and the levels of serum adiponectin（ APN）,interleukin17（ IL-17）,matrix metalloproteinase 9（ MMP-9） and matrix metalloproteinase inhibitor 1（ TIMP-1） were detected before treatment and 6 months after treatment. Results No significant difference was found in FVC,FEV1,FEV1% pred,serum APN,IL-17,MMP-9 and TIMP-1 in the control group as compared with that before treatment（ all P 〉 0. 05）. However,the above indexes all improved in the observation group（ all P〈0. 05） and there were statistically significant differences between the two groups 6 months after treatment（ all P〈0. 05）. Conclusion The Sanzi Zhike capsule can improve the lung function in patients with stable COPD,and the mechanism may be that Sanzi Zhike capsule can decrease the levels of the serum APN,IL-17,MMP-9 and TIMP-1.
