血清脂蛋白a、胱抑素C和尿酸的联合检测在高血压并发脑梗死早期诊断中的意义

摘　　要：目的探讨联合检测脂蛋白a[LP(a)]、胱抑素C(Cys-C)和尿酸(UA)在高血压并发脑梗死诊断中的价值。方法分别采用胶乳增强免疫透射比浊法和URO-PAP法检测85例高血压并发脑梗死患者、71例单纯高血压患者及80例健康者血清中LP(a)、Cys-C和UA的含量。结果 LP(a)、Cys-C和UA在高血压并发脑梗死患者血清中的浓度均高于单纯高血压组和正常对照组,差异有统计学意义(P<0.05);且高血压并发脑梗死组LP(a)、Cys-C和UA阳性率高于单纯高血压组,LP(a)、Cys-C和UA 3项指标联合检测阳性率明显高于各指标单项检测,差异有统计学意义(P<0.05);且高血压并发脑梗死组血清3项指标表达水平两两之间均呈正相关(P<0.05)。结论 LP(a)、Cys-C和UA联合检测可显著提高高血压并发脑梗死诊断的阳性率,对于脑梗死的早期预防和诊断具有十分重要的临床指导意义。Objective To study the clinical application value of combined detection of serum lipoprotein（ a） [LP（ a） ],cystatin-C（ Cys-C） and uric acid（ UA） in patients with hypertension complicated with cerebral infarction. Methods The serum levels of LP（ a） [LP（ a） ],Cys-C and UA in 85 hypertension complicated with cerebral infarction patients,71 simple hypertension patients and 80 healthy controls were detected by the latex enhanced immunoturbidimetry method and URO-PAP method,respectively. Results The serum levels of LP（ a）,Cys-C and UA in hypertension complicated with cerebral infarction patients were all significantly higher than those in simple hypertensive patients and health controls,and the differences were statistically significant（ P〈0. 05）. The positive rates of LP（ a）,Cys-C and UA in hypertension complicated with cerebral infarction group were also higher than those in simple hypertension group,moreover,the positive rate of combined determination of the three biomarkers was significantly higher than that of any single biomarker,and the differences were statistically significant（ P〈0. 05）.In addition,the three biomarkers were positively correlated with each other in hypertension complicated with cerebral infarction group. Conclusion The combined detection of LP（ a）,Cys-C and UA can improve the positive rate in the diagnosis of hypertension complicated with cerebral infarction,and has a great clinical significance in the early prevention and diagnosis of acute cerebral infarction.
