摘　　要：目的探讨实施以社区为基地的预防糖尿病盲的综合干预的效果。方法选择2014年10月至2015年10月包头市社区卫生服务中心慢性病管理登记的216例糖尿病患者为研究对象,采取社区动员、一对一咨询指导、同伴支持小组活动、社会服务组织合作、社区眼病筛查义诊、社区眼健康倡导,大众媒体和新媒体的眼健康信息传播、全科医生和慢病管理人员培训等综合干预措施,连续干预1年。对干预前后与糖尿病视网膜病变和糖尿病盲相关的知识知晓率、对防治糖尿病盲的信念,及进行定期糖尿病眼底检查预防糖尿病盲的行为改变率进行比较分析。结果实施综合干预后,糖尿病患者对糖尿病视网膜病变和糖尿病盲知识的知晓率、防治糖尿病视网膜病变的正确信念持有情况显著改善,干预前后比较,差异有统计学意义(P﹤0.01),干预后进行定期糖尿病眼底检查的行为改变率较干预前有明显提高(P﹤0.01)。结论预防糖尿病盲的社区综合管理模式在促进糖尿病眼健康中效果良好,长期效果还需要观察。Objective To evaluate health management effectiveness on diabetic blindness prevention in Baotou communities. Methods To combine qualitative and quantitative research methodology, to investigate registered 286 cases of diabetes patients in 3 community health service centers in Baotou. Results After implementing community based comprehensive intervention activities in communities, the informative rate of the knowledge, the rate of accurate beliefs and the behavior of acting regular eye check on diabetic blindness prevention have been raised significantly（P﹤0.01）. Conclusion Comprehensive community intervention on diabetic blindness prevention has been achieved better performance, which is appropriate to promote eye health among diabetes patients. And long term observation needs to be carried out as well.
