凝血及纤溶系统与慢性乙型肝炎病毒感染状态的临床相关性分析

摘　　要：目的探讨凝血、纤溶和纤维化与慢性乙型肝炎病毒感染状态的临床相关性,为临床诊治慢性乙型肝炎病毒感染状态提供理论指导依据。方法选取2013年11月-2015年10月于医院接受治疗的慢性乙型肝炎患者150例为研究对象,经检测结果将其分为免疫耐受组50例、免疫清除组50例和非活动组50例,另外选取正常成年人50名作为对照组,分别检测4组受试者的纤溶、凝血功能以及纤维化水平,并统计该3种指标与感染状态的相关性。结果与对照组相比,其他3组的纤维蛋白原(FIB)明显偏低,而PT、TT、APTT等指标则明显上升,差异有统计学意义(P<0.05);与免疫耐受组及非活动组相比,免疫清除组患者3项指标均出现明显变化,差异有统计学意义(P<0.05)。结论纤溶、凝血功能以及纤维化等指标水平含量的变化与慢性乙型肝炎病毒感染的状态呈相关性,可对该疾病的诊断及发展监测提供一定的参考依据。OBJECTIVE To explore the clinical relationship between the coagulation,fibrinolysis,fibrosis,and state of chronic hepatitis B virus infection so as to provide theoretical basis for clinical diagnosis and treatment of the chronic hepatitis B virus infection.METHODS A total of 150 patients with chronic hepatitis B who were treated in hospitals from Nov 2013 to Oct 2015 were recruited as the study objects and divided into the immune tolerance group with 50 cases,the immune clearance group with 50 cases,and the inactive group with 50 cases according to the detection results;meanwhile,50 healthy adults were assigned as the control group.The functions of fibrinolysis and coagulation and level of fibrosis were detected for the four groups of participants,and the correlation between the three indicators and the state of infections was statistically analyzed.RESULTS As compared with the control group,the level of fibrinogen（FIB）of other three groups,with statistical significance（P〈0.05）.As compared with the immune tolerance group and the inactive group,the three indicators of the immune clearance group showed significant changes（P〈0.05）.CONCLUSIONThe functions of fibrinolysis and coagulation and level of fibrosis are associated with the state of chronic hepatitis B virus infection,which can provide guidance for diagnosis of the disease and surveillance of development of illness condition.
