摘　　要：目的研究直肠癌患者腹腔镜术后感染发生率及炎症介质的表达影响,以达到有效控制患者术后感染的目的。方法选取2013年9月-2015年10月于医院行手术治疗的98例直肠癌患者为研究对象,将其随机分为对照组和观察组各49例,对照组患者进行开腹手术治疗,观察组则进行腹腔镜手术治疗,比较两组患者术后感染率,并比较两组患者术前及术后1、3、5d的促炎介质及抑炎介质血清水平。结果观察组患者术后感染率为4.08%,明显低于对照组的18.37%;观察组患者术后1、3、5d的促炎介质及抑炎介质血清水平均低于对照组,差异均有统计学意义(P<0.05)。结论直肠癌患者腹腔镜术后感染率相对更低,对患者的炎症介质也有积极控制作用,因此,对机体不良炎性反应控制效果更好。OBJECTIVE To study the influence of the postoperative infection rates and expression of inflammation mediators in patients with rectal carcinoma after laparoscopic operation,so as to achieve the goal to effectively control the infection.METHODS A total of 98 patients with rectal carcinoma in hospital from Sep.2013 to Oct.2015 were selected as study objects,and they were randomly divided into control group and observation group（each n=49）.Control group were treated with open operation,and observation group were treated with laparoscopic operation,then postoperative infection rates of two groups were compared,and the serum levels of proinflammatory mediators and anti-inflammatory mediators of two groups before the operation and at 1st,3rd and 5th day after the operation were compared.RESULTS The postoperative infection rate of observation group was 4.08%,obviously lower than 18.37% of control group,and the serum levels of proinflammatory mediators and antiinflammatory mediators of observation group at 1st,3rd and 5th day after the operation were all lower than those of control group,and the differences were all significant（all P〈0.05）.CONCLUSION The postoperative infection rate of patients with rectal carcinoma after laparoscopic operation is relatively lower,and the operation type also has active control role for the inflammation mediators of patients,so it has better control role for the bad inflammatory reaction of body.
