耐多药肺结核患者依从性调查及干预措施

摘　　要：目的探讨治疗依从性对耐多药肺结核病患者的相关影响,为提高耐多药肺结核病患者的治疗提供科学依据。方法选择2014年2月-2015年7月在医院接受治疗的112例耐多药肺结核病患者作为研究对象,对其进行用药依从性的调查;将患者分为干预组与对照组,每组各56例,对两组患者的医嘱执行率、痰菌转阴率及不同时间治愈率进行比较,应用SPSS19.0软件对所有的数据进行统计分析。结果 112例耐多药肺结核患者对药物治疗的依从性很低;干预后患者医嘱执行率明显高于对照组患者,两组之间差异有统计学意义(P<0.05);经过治疗后两组患者痰菌转阴率均较稳定,干预组患者在各个时间段的痰菌转阴率均高于对照组,两组转阴率比较差异有统计学意义(P<0.05);干预组患者的治愈率显著高于对照组,两组比较差异有统计学意义(P<0.05)。结论医护人员对患者制定相应的干预方法,以提高患者的治愈率和依从性。OBJECTIVE To investigate the influence of treatment compliance related to MDR-TB（multidrug resistant pulmonary）patients,and to provide scientific basis for improving treatment for MDR-TB patient.METHODS From Feb.2014-Jul.2015,112MDR-TB patients were included as research subjects,and their medication compliance were investigated.All the patients were randomly divided into control and intervention groups,with 56 patients in each group.The rates of compliance,sputum negative conversion rates,and cure rates in different time were compared.Data were statistically analyzed with SPSS 19.0.RESULTS The 112 MDR-TB patients showed very low compliance in medication.The rate of medication compliance in intervention group was significantly higher than control group（P〈0.05）.After treatment,the sputum negative conversion rates in both groups were stable.But the sputum bacterium conversion rates of intervention group at each time period were significantly higher than those in control group（P〈0.05）.The cure rate in intervention group was significantly higher than control group（P〈0.05）.CONCLUSION Medical staffs should came up with relevant intervention methods,so as to improve the cure rate and patient compliance.
