摘　　要：目的 探讨经尿道2 μm激光内切开术联合肾筋膜扩张器尿道扩张治疗小儿尿道下裂术后吻合口狭窄的效果.方法 2011年3月至2015年2月间对诊治的24例尿道下裂术后吻合口狭窄患儿应用2μm激光行内切开术联合肾筋膜扩张器尿道扩张治疗,观察其临床效果.结果 21例手术成功.拔除尿管后均能自主排尿,术后3个月最大尿流率（17.2±2.5）ml/s,平均尿流率（10.6±2.4） ml/s,平均尿流时间（10.6±2.4）s,较术前有明显改善,差异有统计学意义（P均＜0.05）.结论 应用经尿道2μm激光内切开术联合肾筋膜扩张器尿道扩张治疗小儿尿道下裂术后吻合口狭窄具有出血少、围术期安全性高、并发症少等优点,而且操作安全简便,是较为理想的微创手术治疗方法.Objective To explore the efficacy of 2μm continuous wave laser urethrotomy combined with renal sheath dilators for treatment of anastomotic stenosis after hypospadias repairments in pediatrics.Methods From March 2011 to February 2015,24 cases children of anastomotic stricture after hypospadias repair were treated by 2μm continuous wave laser urethrotomy combined with renal sheath dilators,the clinical efficacy was reviewed.Results The treatments were successfully performed on 21 cases.All these patients got fluent urination immediately after the catheter removal,the maximum urinary flow rate of 3 months after operation was （17.2±2.5） ml/s,mean urinary flow rate was （10.6±2.4） ml/s,mean urinary flow time was （10.6±2.4） s,with a great improvement of uroflowmetry,and the difference was significant （P〈0.05）.Conclusion 2μm continuous wave laser combined with renal sheath dilators is a superior micro-invasive surgery method for children with anastomotic stenosis after hypospadias repairments,which combined with little blood loss,high safety,convenient operation and infrequent complications.
