经尿道前列腺电切术与开放性手术对良性前列腺增生患者性功能的影响分析

摘　　要：目的:探讨经尿道前列腺电切术与开放性手术对良性前列腺增生患者性功能影响性,以期提高临床诊治水平。方法:选取2013年2月到2015年2月于我院就诊的98例良性前列腺增生患者为研究对象,分成两组,对照组49例,予开放性手术治疗,观察组49例,予经尿道前列腺电切术,观察治疗后两组相关指标变化情况。结果:对照组射精疼痛发生率为10.2%、逆行射精发生率为26.53%、ED发生率为30.61%,观察组射精疼痛发生率为2.04%、逆行射精发生率为16.33%、ED发生率为12.25%,两组比较差异均有统计学意义(P<0.05),而在精量增加、精量减少上比较差异则无统计学意义(P>0.05)。两组治疗后3、6、12个月在IPSS评分上较治疗前评分均下降,治疗前和治疗后3、6、12个月IPSS评分比较差异有统计学意义(P均<0.05),治疗后3个月、6个月、12个月IPSS评分比较差异均无统计学意义(P>0.05)。两组治疗后3、6、12个月在IIEF-5评分上较治疗前评分均下降,治疗前和治疗后3、6、12个月IIEF-5评分比较差异有统计学意义(P均<0.05),治疗后3个月、6个月、12个月IIEF-5评分比较差异均有统计学意义(P<0.05)。结论:经尿道前列腺电切术治疗良性前列腺增生对患者性功能影响性小。Objectives： To study the impact of different treatment options on the sexual function of patients with benign prostatic hyperplasia in order to improve the clinical diagnosis and treatment. Methods： 98 benign prostatic hyperplasia patients from February 2013 to February 2015 were divided into two groups,the control group of 49 patients for open surgery,and the observation group of 49 cases for the transurethral resection. Changes in relevant indexes of the treatment groups were observed. Results： In control group,the incidence of painful ejaculation,retrograde ejaculation and ED was 10. 2%,26. 53% and 30. 61%,while in the observation group,those were2. 04%,16. 33% and 12. 25 % respectively,with statistically significant difference（ P〈0. 05）. Difference in the increase / reduce in ejaculate volume was not statistically significant（ P〈0. 05）. The IPSS score 3,6 and 12 months after treatment all decreased compared with those before treatment in both groups,with statistically significant difference（ P〈0. 05）,but no significant difference was found between that at 3rd,6thand 12thmonth（ P〈0. 05）. The IIEF- 5 score 3,6 and 12 months after treatment all decreased compared with those before treatment in both groups,with statistically significant difference（ P〈0. 05）,so as the difference between that at 3rd,6th and12 th month（ P〈0. 05）. Conclusion： Transurethral resection has little impact on the sexual function of patients with benign prostatic hyperplasia.
