摘  要:目的:对硝酸咪康唑与制霉素治疗妊娠合并念珠菌性阴道炎的临床效果进行对比分析,指导临床用药。方法:将113例患者随机分为两组:A组患者在使用2%NaHCO_3溶液清洗外阴和阴道后采用硝酸咪康唑400mg置入阴道后穹窿,连续3晚;B组患者在使用2%NaHCO_3溶液清洗外阴和阴道后采用制霉素10万U置入阴道后穹窿,连续3晚。观察和比较两组患者的一般情况、临床疗效、不良反应、复发率和新生儿娩出后皮肤真菌携带率。结果:两组患者一般情况、临床疗效、复发率和新生儿皮肤真菌携带率均无统计学差异(P>0.05);不良反应发生率制霉素治疗组高于硝酸咪康唑治疗组(P<0.05),但两组均未发现因不能耐受不良反应而放弃治疗的病例。结论:硝酸咪康唑和制霉素经阴道局部给药应用于妊娠合并念珠菌性阴道炎的治疗,其治疗的有效性、安全性和复发率无明显差异。且疗效显著,对胎儿无明显影响,且可以降低胎儿感染几率,同时降低新生儿皮肤真菌携带率,用药方便,可以在临床推广使用。Objectives: To compare the clinical effect of the miconazole nitrate and nystatin in the treatment of pregnancy with vaginal yeast infection,to guide the clinical medication. Methods: 113 patients were randomly divided into two groups: group A,after the use of 2% NaHCO_3 solution cleaning the vulva and vagina,had 400 mg of miconazole put into vaginal fornix for three consecutive night; group B,after the use of 2% Na HCO_3 solution cleaning the vulva and vagina,had 100000 U of nystatin put into vagina fornix for three consecutive night. The general condition,clinical curative effect,adverse reaction,recurrence rate and the fungus carry on the neworns' skin was observed and compared between the two groups. Results: Difference in general condition,clinical curative effect,recurrence rate and the newborn skin fungi carry rate was not statistically significant( P〈0. 05). The adverse reaction of nystatin treatment group was higher than that of the miconazole treatment group. Conclusion: Miconazole and nystatin has no significant difference in the treatment of pregnancy with vaginal yeast infection. Safe and effective,the treatment can reduce fetal infection rate,and decrease the rate of neonatal skin fungi carry,which can be used widely in clinical.
