不同中医辨证分型慢性鼻-鼻窦炎患者鼻内窥镜术后中医药治疗的临床研究

摘　　要：目的:观察不同中医辨证分型慢性鼻-鼻窦炎患者鼻内窥镜术后中医药治疗的临床疗效。方法:选取2014年2月-2015年6月于本院进行鼻内窥镜手术治疗后的100例慢性鼻-鼻窦炎患者作为研究对象,随机分为对照组和观察组,每组50例。对照组和观察组均进行术后常规治疗,观察组加用中医辨证分型治疗12周。术后6个月,比较两组Ⅰ型、Ⅱ型、Ⅲ型患者的治疗总有效率、上皮化时间、清洁时间及不同中医辨证分型的疗效。结果:观察组Ⅰ型、Ⅱ型及Ⅲ型患者的治疗总有效率均高于对照组,上皮化时间和清洁时间均短于对照组,比较差异均有统计学意义(P<0.05)。两组肺经蕴热型和肺脾气虚型的疗效比较差异均有统计学意义(P<0.05)。两组湿热型疗效比较差异无统计学意义(P>0.05)。结论:慢性鼻-鼻窦炎患者鼻内窥镜术后运用中医辨证治疗的临床疗效较好,肺经蕴热型和肺脾气虚型患者辨证服用中药辅助治疗能取得较好疗效,中医药辨证治疗对于慢性鼻-鼻窦炎患者术后的康复应用价值较高。Objective：To observe the clinical effect on traditional Chinese medicine（TCM）of syndrome differentiation and treatment on chronic nasal sinusitis after endoscopic sinus surgery（ESS）.Method：From February 2014 to June 2015,100 cases of chronic nasal sinusitis after endoscopic sinus surgery in our hospital were selected as the research objects,they were randomly divided into the control group and the observation group,each group had 50 cases.The two groups were treated with routine treatment,the observation group were targeted treatment according to TCM of syndrome differentiation and treatment for 12 weeks,after 6 months,the total effective rate,epithelial time,cleaning time of TypeⅠ,type Ⅱ and type Ⅲ patients in two groups were compared,and the effect by TCM of syndrome differentiation and treatment of two groups were compared.Result：The total effective rates of observation group with typeⅠ,typeⅡ and type Ⅲ were all higher than the control group,the differences were statistically significant（P〈0.05）.The effect of syndrome of wind-heat on lung and the syndrome of qi deficiency on the lung and pleen,the differences were statistically significant（P〈0.05）.The effect of syndrome of damp and heat show insignificance（P〈0.05）.Conclusion：Chinese herbal medicine（CHM） treatment may improve the effect of sinus surgery,particularly in syndrome of wind-heat on lung and the syndrome of qi deficiency on the lung and pleen.
