Ishiguro法和钢丝抽出法治疗陈旧骨性槌状指短期疗效对比

摘　　要：目的：对比Ishiguro法和钢丝抽出法治疗陈旧骨性槌状指的短期临床疗效。方法：自2010年9月～2014年2月我院收治的38例陈旧骨性槌状指患者，术中C臂透视下对撕脱骨块进行翘拨闭合复位，对其中20例可复位的槌状指采用Ishiguro法进行手术并入A组随访，无法闭合复位的18指采用切开复位钢丝抽出法手术治疗并入B组随访。随访记录时间为术后8个月～16个月，平均13．7个月，采用Dargan法进行患指功能评定，采用患指侧位X片对比评估远指间关节主活动范围，同时记录骨愈合情况。结果采用SPSS17．0统计软件进行分析得出结论。结果：采用Dargan法进行功能评定：A组优良率：80％；B组优良率：55．56％，2组存在统计学差异，认为A组功能恢复优于B组；患指远指间关节活动度：A组平均43．2°，B组平均32．8°，2组存在统计学差异，认为A组关节活动度优于B组；骨愈合情况：A组2指未愈合；B组4指未愈合，2组无统计学差异；同时A组已愈合的18指的平均愈合时间为8．8周，B组14指的平均愈合时间为8．3周，2组无统计学差异，认为A、B2组骨折愈合情况无明显差异。结论：短期随访支持闭合复位Ishiguro法较切开复位钢丝抽出法对患指功能和远指间关节主动活动度恢复更优，骨折愈合情况无明显差异。Objective： To evaluate the short - term clinical efficacy of Fresh osseous mallet finger operated in Ishiguro method com- pared with pull - out wire. Methods： From September 2010 to February 2014, 38 patients in our hospital with fresh osseous mallet finger, were randomly divided into A and B group. 20 patients of group A were operated by Ishignro method. 18 patients of group B were operated by open reduction and pull - out wire method. Recording time of follow - up was 8 months to 16 months postoperatively, an average of 13. 7 months. Dargan method is adopted for functional assessment, and X - ray is adopted to assess the range of motion of DIP and record the time of bone healing. Conclusion using SPSS17.0 statistical software was used to analyze the results. Results： Functional assessment： Fine rate of group A was 80% but 55.56% in group B. Group A was statistically significantly better than group B （ P 〈 0.05）. The range of motion of DIP： average of 43.2° in group A and 32.8 ° in group B Group A was statistically significantly better than group B （ P 〈0. 05 ）. The rate and the time of bone healing ： There was no statistically significantly difference in two groups （ P 〉 0.05 ）. Conclusion： The short - term follow - up supported of Ishignro method was statistically significantly better than pull - out wire method in functional recovery of DIP , but there was no statistically significantly difference in the rate and the time of bone healing.
