四妙五苓散加味治疗急性痛风性关节炎的临床观察

摘　　要：目的观察四妙五苓散加味治疗急性痛风性关节炎的临床疗效。方法将60名急性痛风性关节炎患者分为两组,治疗组给予四妙五苓散加味治疗,对照组给予口服秋水仙碱治疗,观察时间为2周。比较两组患者的临床疗效,观察治疗前后局部症状的改善情况及C反应蛋白、血沉、尿酸的变化。结果治疗组总有效率明显优于对照组(P<0.05)。治疗后,两组关节疼痛、压痛、红肿、活动受限积分均较治疗前明显降低(P<0.01);治疗组疼痛、压痛、红肿积分显著低于对照组(P<0.01)。治疗后,两组C反应蛋白、血沉及尿酸水平均明显降低(P<0.01),其中治疗组血沉及尿酸水平降低程度大于对照组(P<0.01)。结论四妙五苓散加味治疗急性痛风性关节炎有很好的疗效,可以显著改善局部症状,降低急性期反应标志物和尿酸水平,值得在临床中进行深入的研究和推广。Objective To observe the clinical efficacy of modified Simiao Wuling powder decoction in treatment of acute gouty arthritis.Methods 60 patients with acute gouty arthritis were radomly divided into treatment group and control group. Treatment group was treated with Simiao Wuling powder decoction,control group were treated with oral colchicines. After 2 weeks of observation,the clinical efficacies were compared between the two groups,and improved conditions of local symptoms as well as changes of C- reactive protein（ CRP）,erythrocyte sedimentation rate（ ESR） and uric acid（ UA） were observed. Results The overall response rate of treatment group（ 90%） was superior to that of control group（ 76. 67%）（ P〈0. 05）. After treatment,the scores of joint pain,pressure pain,red swelling and limited movement in both groups decreased significantly compare with those before treatment（ P〈0. 01）,the scores of joint pain,pressure pain and red swelling in treatment group were lower than those in control group dramatically（ P〈0. 01）. After treatment,the levels of CRP,ESR and UA in both groups reduced obviously（ P〈0. 01）,in which the decreased range of ESR and UA in treatment group was larger than that of control group with statistical significance（ P〈0. 01）. Conclusion Simiao Wuling powder decoction can effectively ameliorate the local symptoms of patients with acute gouty arthritis,decrease reactive biomarkers in acute phase and UA level with obvious efficacy,is worthy of being studied and promoted in clinical application.
