针刀联合中药治疗急性期不同程度腰椎间盘突出症临床研究

摘　　要：目的观察针刀联合中药治疗急性期不同程度腰椎间盘突出症的疗效。方法将240例患者随机按照临床分度分为3度,各两组即治疗组(针刀联合中药组)120例和对照组(牵引+西药组)120例。按照不同分度采用不同针刀手法治疗每周1次,4次为1疗程联合中药,每日1剂。对照组应用牵引联合西药:牵引1周3次,连续4周为1疗程。西药以消炎镇痛药为主,如塞来昔布片,每日1次,每次1粒。分别以VAS、JOA、表面肌电图等为观察指标,在治疗前后分别记录结果。结果共纳入研究病例233例,治疗组总有效率为96.64%,高于对照组的72.81%(P<0.01),治疗前后的VAS评分、JOA评分针刀联合中药组与对照组相比较差异显著(P<0.01),两组治疗前后表面肌电图有效率为79.49%和53.59%,差异有统计学意义(P<0.01)。结论针刀联合中药治疗不同程度急性期腰椎间盘突出症患者,疗效确切,治疗周期短。Objective： To observe the curative effect of acupotomology combined Chinese medicinal herbs on different degree therapy on lumbar disc herniation in acute stage. Methods： 240 cases were divided into the treatment group（acupotomy combined Chinese medicinal herbs）（n = 120）and the routine treatment control group（traction and western medicine）（n = 120） for three stages and three grades using randomized,controlled,blinded study methods. The treatment groups was treated with acupotomy combined Chinese medicinal herbs with different Methods,based on different grades. The acupotomy was taken once a week and four times as a course of treatment. Meanwhile,Chinese medicinal herbs was taken once per day. The control group was treated with traction and western medicine. The traction was taken 3 times a week and four weeks as a course of treatment. The western medicine was given priority to with anti-inflammatory analgesic,for example celecoxib,once a day,1 grain at a time. The curative effect of all groups was analyzed. VAS score,JOA score and s EMG were recorded in selected patients pre-operatively and post-treatment. Results： Among 233 cases,total effective rate of the treatment group was 96. 64%,the control group 72. 81%;the difference was significant（P〈0.01）. Before and after treatment,there was significant difference in VAS score and JOA score（P〈0.01）. Referring to the s EMG effective rate,the two groups was 79.49% and 53.59% before and after treatment. The difference was significant. Conclusion： Acupotomy combined Chinese medicinal herbs has a exactly curative effect on the treatment of acute lumbar disc herniation with different methods,based on different grades. The treatment period is short and the therapeutic method was easy to promote clinically.
