摘 要:目的探讨3种不同胰岛素泵基础量分段法对2型糖尿病患者血糖控制的效果。方法选取2010年7月至2014年7月于延安大学附属医院就诊的180例2型糖尿病患者作为研究对象,按随机数字表法分为3组,每组60例,分别是3段组、5段组和24段组。比较三组患者治疗前后血糖达标时间、血糖值、低血糖发生率以及胰岛素用量的差异。结果早饭后三组血糖水平均上升,午饭后、晚饭后、凌晨3点各组血糖水平呈下降趋势,3组患者血糖值差异无统计学意义(P>0.05)。5段组和24段组治疗时间、胰岛素用量少于3段组[(3.6±0.3)d、(2.9±0.7)d比(5.5±0.8)d,(41±5)U、(36±4)U比(43±6)U],24段组治疗时间、胰岛素用量少于5段组,差异有统计学意义(P<0.05)。3段组低血糖发生率为13.3%(8/60),5段组为6.7%(4/60),24段组为3.3%(260),3组治疗方法的低血糖发生率比较差异无统计学意义(P>0.05)。结论 3种不同胰岛素泵基础量分段法对2型糖尿病患者血糖控制的效果相近,但综合分析5段法的设置更为简单方便、疗效好。Objective To investigate the blood sugar control effect of three insulin pump basal rate segment methods for type 2 diabetes. Methods A total of 180 patients with type 2 diabetes in Affiliated Hospital of Yan'an University from Jul. 2010 to Jul. 2014 were included in the study and divided into three groups according to the random number table method ,60 eases each :3-segment group ,5-segrnent group and 24-segment group respectively. The blood glucose reaching standard time, blood glucose level, the incidence of hypoglycemia and insulin dosage of the three groups were compared. Results After breakfast blood glucose levels rose,then decreased after lunch, after dinner,3:00 in the morning in each group,with no significant difference ( P 〉 0. 05 ). The treatment time and insulin usage of the 5-segmentgroup [ ( 3.6 ± 0. 3 ) d, (41± 5 ) U ] and 24-segment group [ (2.9 ± 0. 7 ) d, (36 ±4 ) U ] were less than the 3-segment group [ ( 5.5 ±0. 8 ) d, (43 ± 6 ) U ], and the 24-segment group less than the 5-segment group, with statistically significant difference ( P 〈 0. 05 ). The hypoglycemia incidence of the 3-segment group was 13.3 % ( 8/60 ), of the 5-seg- ment group was 6. 7% (4/60) ,of the 24-segment group was 3.3% (2/60) ,the difference was not statistical- ly significant ( P 〉 0. 05 ). Conclusion The three insulin pump basal rate segment methods to control blood sugar for type 2 diabetes have similar effect, but comprehensively speaking the 5-segment method is more simple and convenient with good effect.
