腰硬联合麻醉镇痛对母婴结局的影响

摘　　要：目的观察腰硬联合麻醉镇痛对母婴结局的影响。方法选取2014年6～12月在北京市房山区妇幼保健院进行阴道试产的产妇500例。依据随机数字表法将产妇分为镇痛组（186例）和对照组（314例），镇痛组使用腰硬联合麻醉镇痛；分析两组患者的产程、活跃期及产妇及新生儿的结局。结果镇痛组产妇的总产程、活跃期及第二产程均显著长于对照组[（960．2±23．1）min比（780．8±18．3）min、（420．3±30．2）rain比（300．4士25；3）min及（88．3±5．2）min比（56．9土3．8）min，P〈0．01]；产后2h两组产妇的出血量及尿潴留发生率、自然分娩率、羊水污染达Ⅱ度及Ⅲ度的发生率和新生儿出生后1min及5minApgar评分≤7分的发生率差异无统计学意义（P〉0．05）。结论应用腰硬联合麻醉镇痛会延长产妇的分娩时间，但对产妇母婴结局的影响并不明显。Objective To observe the effects of spinal epidural combined anesthesia on maternal and infant outcomes. Methods Total of 500 puerpera for virginal trial childbirth in Beijing Fangshan District Maternity ＆ Child Healthcare Hospital from Jun. 2014 to Dec. 2014 were included in the study. The puerperas were divided into an analgesia group and a control group according to the random number table method： 186 cases in analgesia group receiving spinal and epidural anesthesia;314 cases in the control group, and the total labor duration,the active period and the maternal and infant outcomes of the two groups were analyzed. Results The total labor duration,the active period the second labor of the analgesia group were significantly longer than the control group [ （960. 2 ± 23. 1 ） min vs （780. 8 ± 18. 3 ） rain, （ 420. 3 ± 30. 2） min vs （ 300. 4 ± 25.3 ） min, （88. 3 ± 5. 2） min vs （56. 9 ± 3.8 ） min, P 〈 0. 01 ]. There was no statistical difference between the two groups in postpartum 2 h maternal bleeding,the incidence of urinary retention,the natural birth rate, the incidence of degreeⅠ and Ⅱ amniotic fluid meconium contaminatian,and the incidence of neonatal Apgar score ≤ 7 scores 1 and 5 min after birth （ P 〉 0. 05 ） Conclusion Spinal epidural combined anesthesia extends the puerpera childbirth time, but does not affect the maternal and infant outcomes obviously.
