更昔洛韦联合强的松及人免疫球蛋白治疗中老年重症带状疱疹疗效评价

摘　　要：目的探讨更昔洛韦联合强的松及人免疫球蛋白治疗中老年重症带状疱疹的疗效。方法选择重症带状疱疹患者80例,按就诊顺序随机分为2组。观察组42例,予更昔洛韦联合强的松[0.5 mg/（kg·d）]及小剂量人免疫球蛋白（2.5~5 g）治疗;对照组38例,不加人免疫球蛋白,其余治疗同观察组。观察并比较两组患者的皮损止疱时间、开始结痂时间、完全脱痂时间、住院时间,治疗第4天、第8天及1个月、2个月时的疼痛情况（VAS评分）及疗效。结果观察组较对照组在皮损止疱时间、开始结痂时间、完全脱痂时间、住院时间均明显减少（t=4.93,6.12,2.82,3.20,P〈0.05）;观察组较对照组在治疗第4天、第8天、1个月、2个月时的VAS评分上明显减少（t=2.40,3.34,3.12,3.31,P〈0.05）,后遗神经痛减少;观察组总有效率明显高于对照组（90.48%比73.68%,χ2=3.90,P〈0.05）。结论更昔洛韦联合强的松及小剂量人免疫球蛋白可明显提高中老年重症带状疱疹患者的疗效,促进皮损恢复,减少后遗神经痛的发生。Objective To evaluate the efficacy of ganciclovir combined with prednisone and human immunoglobulin in treating serious herpes zoster in middle- aged and elderly patients. Methods The patients with serious herpes zoster were randomly divided into two groups in accordance with the order of treatment. The observation group（42 patients） was treated with ganciclovir plus prednisone [0. 5 mg/（kg ~ d） ] combined with low dose of human immunoglobulin （2.5 - 5 g） ; the control group （38 patients） was treated without using human immunoglobulin. The treatment efficacy, and the time of stopping blister, beginning scab, complete decrustation and hospital- ization time, as well as the visual analogue scale（VAS） score after treatment of 4, 8 d and 1, 2 month were observed and com- pared. Results The time of the observation group in stopping blister, beginning scab, complete decrustation and hospitalization was obvi- ously shorter than the control group（t =4. 93,6. 12, 2.82, 3.20, P 〈 0. 05）. The VAS score of the observation group was obviously low- er than the control group after treatment of 4, 8 d and 1, 2 month（ t = 2. 40, 3.34, 3.12, 3.31, P 〈 0. 05）. The neuralgia incidence was also obviously reduced in the observation group, and the effective rate of the observation group was obviously higher than that of the control group（90.48% vs 73.68%, X2 =3.90, P 〈 0. 05）. Conclusion The efficacy is obviously increased, neuralgia incidence is low- ered, and the recovery of skin lesions is accelerated by using ganciclovir combined with prednisone and low dose human immunoglobulin to treat serious herpes zoster in middle- aged and elderly patients.
