卵巢子宫内膜异位囊肿的CT诊断和鉴别诊断

摘　　要：目的：总结分析卵巢子宫内膜异位囊肿的CT表现,以提高其CT的诊断和鉴别诊断水平。材料与方法：回顾性分析177例经手术和病理证实的卵巢子宫内膜异位囊肿的临床和CT表现,并收集同期卵巢其它囊性病变作对比。所有病例均行CT平扫＋增强扫描。结果：单侧发病131例,双侧46例,共224个囊肿,平均最长径6.2±2.0cm;单房囊肿74个,多房囊肿150个,其中姐妹囊9个,外子囊（卫星囊）的91个,内子囊6个,混合型46个;圆形或卵圆形163个,不规则形61个;囊液密度2~68Hu,平均24.3±7.9Hu,11内见高密度出血灶;厚壁11个,薄壁或未显示17个,厚薄不均196个,钙化4个;外缘毛糙,与周围组织分界不清楚202个,清楚22个。结论：卵巢子宫内膜异位囊肿的CT表现有一定的特征性,结合临床,大部分能做出明确的CT诊断。Purpose ： To summerize CT findings of ovarian Endometriotic cyst so as to improve the diagnosis and dif- ferential diagnosis level of this disease. Materials and Methods： Clinical data and CT findings in 177 patients （224 cysts） with Ovarian Endometriosis cyst confirmed by surgery and pathology were analysed. All the patients accepted plain CT scan and enhanced CT scan. Results： Of the 224 cysts, the shape was oval or round in 163 cysts, irregular in 61 cysts, gourd- shaped in 9 cysts. The size of the cysts ranged from 1.0cm to 12. 0cm, the mean diameter was 6. 2 ±2.0cm. Uniloculor and multiloculor forms were seen respectively in 74 and 150 of 224 Ovarian Endometriosis cysts. And sister loeuli, satellite sign （out son loculi） , inner son loculi and mixed forms in 9, 91, 6 and 46 of 150 muhilocular cysts. The density of fluid varied 2 -68Hu with the mean of 24. 2 ± 8.1Hu, 11 cysts had focal hyperdense area. The systic wall was seen in 223 cysts and the wall had different degree contrast enhancement. Thin, nevenly thickened and thickened walls were found respectively in 17, 196 and 11 of 224 cysts. The margins of cyst were sharp in 22 and ill defined in 202. Conclusion： The CT finding of ovarian Endometriosis cysts were to some extent characteristic features and which can help to correct diagnosis or differential diagnosis of it.
