摘  要:目的:利用3.0T动态对比增强磁共振(dynamic contrast-enhanced magnetic resonance,DCE-MR),探讨直肠癌患者容量转移常数(volume transfer constant,Ktrans)、速率常数(rate constant,Kep)以及细胞外血管外间隙容积比(extra-vascular extra-cellular space fractional volume,Ve)与肿瘤组织分化程度的关系。材料与方法:根据术后病理,56例直肠癌患者分为高、中、低分化组,利用单因素方差分析比较各组Ktrans、Kep、Ve值。结果:高、中、低分化组的Ktrans值分别为0.154±0.009/min、0.210±0.027/min、0.365±0.021/min;Kep值分别为0.469±0.008/min、0.528±0.080/min、0.637±0.006/min;Ve值分别为0.739±0.006、0.841±0.070、0.923±0.105,高、中、低分化组间Ktrans、Kep、Ve差异均有统计学意义(P<0.01)。结论:DCE-MR定量参数Ktrans、Kep、Ve值可提供肿瘤微循环信息,并且能够无创性预测直肠癌的恶性程度。Purpose: To explore the relationship between Ktrans, Kep, Ve and tissue differentiation degree of rectal cancer by use 3.0T dynamic contrast-enhanced MR(DCE-MR). Materials and Methods: 56 rectal cancer patients were di- vided into well, moderately and poorly differentiated group according to postoperative pathology. Contrasting Ktrans, Kep, Ve of each group by single factor variance analysis. Results: Ktrans of well, moderately and poorly differentiated group was 0. 154 ±0. 009/min, 0. 210 ±0. 027/min, 0. 365 ±0. 021/min; Kep was 0. 469 ±0. 008/min, 0. 528 ±0. 080/rain, 0. 637 ± 0. 006/min; Ve was 0. 739 ±0. 006, 0. 841±0. 070, 0. 923 ± 0. 105, differences of Ktrans, Kep, Ve of well, moderately and poorly differentiated group were statistically significant (P 〈 0. 01 ). Conclusion: Ktrans, Kep, Ve of DCE-MR quantita- tive parameters can provide information of tumor microcirculation, and predict noninvasively malignant degree of rectal cancer.
