加减温胆汤治疗小儿肠系膜淋巴结炎(痰热互结型)的疗效观察

摘　　要：目的观察理气化痰法治疗痰热互结型小儿肠系膜淋巴结炎的临床疗效。方法 2013年10月至2014年10月安徽中医药大学第一附属医院儿科门诊收治急性肠系膜淋巴结炎(痰热互结型)患儿66例,随机分为对照组和观察组各33例。观察组予以中药加减温胆汤颗粒剂口服,对照组予以头孢丙烯颗粒和酪酸梭菌活菌片口服,疗程为2周。观察两组患儿治疗后总疗效和腹部肠系膜淋巴结大小变化。结果观察组总有效率为90.9%(30/33),显著高于对照组66.7%(22/33),差异有统计学意义(P<0.05)。两组治疗后肠系膜淋巴结横径和纵径较治疗前均显著缩小,差异有统计学意义(P<0.05)。治疗后观察组肠系膜淋巴结横径和纵径均显著低于对照组,差异有统计学意义(P<0.05)。结论加减温胆汤治疗痰热互结型小儿肠系膜淋巴结炎疗效肯定,值得推广。Objective To observe the clinical curative effect of the treatment of regulating qi and eliminating phlegm for phlegm heat type of mesenteric lymphadenitis in children. Methods From October 2013 to 2014,66 children were treated in Department of Pediatrics of the First Affiliated Hospital of Anhui TCM University for acute mesenteric lymphadenitis（phlegm heat stasis type）, and they were randomly divided into the control group and observation group of 33 cases in each. The observation group was treated with tradi- tional Chinese medicine Wendan decoction granules,orally, while control group was given cefprozil granules and Clostridium butyricum live bacteria tablets; treatment lasted for 2 weeks. The total curative effect and the size of abdominal mesenteric lymph nodes were observed in the two groups after treatment. Results The total effective rate of observation group was 90.9 % （30/33） ,which was significantly higher than that of control group （66.7 %, 22/33）, and the difference was statistically significant （ P 〈 0.05）. Mesenteric lymph nodes of the two groups in the transverse and longitudinal diameters were significantly reduced, the difference being statistically significant（P〈0.05）. After treatment, the transverse diameter and longitudinal diameter of mesenteric lymph nodes in the observation group were significantly smaller than those in the control group,and the difference was statistically significant（P〈0.05）. Conclusion The modified Wendan decoction in the treatment of phlegm heat type of mesenteric lymphadenitis in children has definite curative effect, which is worthy of promotion.
