沙利度胺联合液氮冷冻治疗结节性痒疹的效果观察

摘　　要：目的观察沙利度胺联合液氮冷冻治疗结节性痒疹的效果。方法选取2013年1月-2015年6月于本院就诊的结节性痒疹患者54例,将其随机分为观察组和对照组,每组27例。观察组口服沙利度胺50 mg,2次/d,症状改善后逐渐减量为50 mg或25 mg,1次/d,同时用液氮冷冻治疗,根据皮损的大小、厚度、部位以及局部反应可重复多次冻融。对照组予口服地氯雷他定及外涂复方氟米松软膏治疗。结果观察组治疗总有效率为85.18%,复发率为8.70%;对照组总有效率为44.44%,复发率为41.67%。两组有效率和复发率比较,差异有统计学意义（P〈0.01,P〈0.05）。两组均无严重不良反应。结论沙利度胺联合液氮冷冻治疗结节性痒疹方便、有效、不良反应少,具有临床应用价值。Objective To observe the effectiveness of thalidomide combined with cryotherapy in the treatment of prurigo nodularis. Methods Altogether 54 cases of prurigo nodularis patients who had been treated in the hospital from January 2013 to June 2015 were selected and randomly divided into the observation group and control group with 27 cases in each group.Patients in observation group were treated with 50 mg,thalidomide 2 times per day.And 50 mg or 25 mg once daily was administered together with cryotherapy after symptoms were tapering off.The freezing and thawing could be performed repeatedly according to the size,thickness,position and local reactions of lesions.Patients in control group received oral desloratadine and were coated with compound flumetasone ointment. Results The effectiveness rate and recurrence rate of observation group was respectively 85.18% and 8.70%,44.44% and 41.67% in control group,with statistically significant differences between the two groups（P〈0.01,P〈0.05）.And there were no serious adverse reactions in both groups. Conclusion Application of combination of thalidomide and cryotherapy in the treatment of prurigo nodularis demonstrates its advantages in convenience,effectiveness and less adverse reactions,which is of great value in clinical application.
