CT引导下^(125)I粒子植入治疗纵隔转移瘤并发症的观察护理

摘　　要：目的探讨125I粒子植入治疗纵隔转移瘤并发症的观察护理。方法对30例纵隔转移瘤患者采用125I粒子植入治疗,并做好并发症的观察护理。结果 30例患者中并发症有气胸6例(20%),其中需要置胸腔闭式引流管的为3例,另3例气胸量较少,自行吸收;肺出血4例(13.3%);针道出血4例(13.3%);咳血4例(13.3%);恶心呕吐1例(3.3%);疼痛2例(6.7%);过敏反应1例(3.3%),均经治疗后恢复,未出现严重并发症。结论 CT引导下125I粒子植入治疗纵隔转移瘤效果好,安全性高,创伤小.并发症可控。加强125I粒子植入治疗前后的观察和护理,可保证治疗有效率。Objective To investigate the nursing care of the complications following the125 I seed implantation in the treatment of mediastinal metastases. Methods 30 patients with mediastinal metastases were treated with125 I brachytherapy,and complications and records of nursing cares were collected. Results Of the 30 patients studied,there were 6 patients（ 20%） developing pneumothorax,3 of which needed to the thoracic drainage tube. Pulmonary hemorrhage were observed in four cases（ 13. 3%）,and incidence of needle tract hemorrhage was 13. 3%（ 4 /30）. Hemoptysis were found in 4 cases（ 13. 3%）,and 1 case of nausea and vomiting. In addition,chest pain and allergic reaction were also observed in 2 cases and 1 case respectively. All the patients with the above complications were treated properly and recovered effectively after treatment,no serious complications were observed. Conclusion CT guided125 I seed implantation were effective,safe,less invasive and had slightly amount of controllable complications in the treatment of mediastinal metastases. Enhancement of the nursing care before and after the125 I brachytherapy enabled the treatment efficiency.
