摘 要:目的探讨99mTc-DTPA肾动态显像在评估化疗患者早期肾功能损害中的临床应用。方法对我院肿瘤科诊断明确的37位恶性肿瘤首次化疗患者在化疗前后分别进行肾动态显像,测定肾小球滤过率(GFR),判断肾功能受损情况。作为对照,患者当天检测血清肌酐(Scr)、尿素氮(BUN)、尿微量白蛋白(UMA)、尿β2球蛋白(β2-MG)和胱抑素C(Cys C)水平。结果化疗后,患者双肾血流灌注峰值下降,总GFR均值化疗后较化疗前明显下降(P<0.05)。37例患者Scr,BUN,UMA和β2-MG化疗前后差异无统计学意义(P>0.05),而化疗后Cys C水平较化疗前明显增高(P<0.01)。结论核素肾动态显像能早期、客观监测化疗药物对肾功能的损害,是评估肾功能损伤程度的可靠指标。Objective To observe the clinical value of radionuclide renal dynamic imaging in detecting the early changes of renal function in chemotherapy patients. Methods 37 patients with malignant tumor in our hospital were treated with99mTc-DTPA renal dynamic imaging before and after chemotherapy to detect renal dysfunction. The glomerular filtration rates( GFR) were gained,and simultaneously,the results were analyzed along with serum creatinine( Scr) and blood urea nitrogen( BUN),Urine micro-albumin( UMA),β2-microglobin( β2-MG),Cystatin C( Cys C) as control. Results GFR in 37 patients after chemotherapy was obviously decreased in comparison with that before chemotherapy( P〈0. 05). Statistically,there was no significant change in Scr,BUN,UMA and β2-MG( P〉0. 05). However,the level of Cys C was obviously demonstrated that there was a clearly statistic difference before and after chemotherapy( P〈0. 01). Conclusion Radionuclide renal dynamic imaging could be objective and visual to detect the early change of renal function after chemotherapy in patients,and it may be used as reliable index to evaluate the renal function damage.
