摘  要:目的检测寻常型银屑病患者外周血中CD4^+T淋巴细胞上程序性死亡分子-1(PD-1)的表达,探讨PD-1在银屑病发病机制中的作用。方法入组30例寻常型银屑病患者定义为银屑病组,19例健康者为健康对照组。采集银屑病组患者及健康对照者清晨空腹外周静脉血,肝素锂抗凝,采用流式细胞仪检测外周血中CD4^+T淋巴细胞上PD-1分子的表达,比较两组研究对象CD4^+T淋巴细胞上PD-1分子表达的差异,并分析其与银屑病皮损面积和严重程度指数(PASI)评分及病程的相关性。结果银屑病组CD4^+T淋巴细胞上PD-1分子的表达量为(14.33±5.16)%,明显低于健康对照组的(21.63±4.70)%,差异有统计学意义(P<0.05),银屑病组CD4^+T淋巴细胞上PD-1分子与PASI评分呈负相关(r=0.52,P=0.003),但与病程无明显相关性(r=0.09,P=0.650)。结论银屑病患者CD4^+T淋巴细胞上PD-1分子的表达比健康人群明显降低,且与疾病严重程度相关。Objective To investigate the expression of programmed death 1(PD-1)molecular on CD4^+T cells in peripheral blood of patients with psoriasis,and to explore its role in the pathogenesis of psoriasis.Methods Totally 30 patients with vulgaris psoriasis and 19 health controls were enrolled in present study.Peripheral blood was obtained from patients and health controls.The expressions of PD-1molecular on CD4^+T cells in peripheral blood were measured by flow cytometry.The expressions of PD-1 molecular on CD4^+T cells between patients and health controls were analyzed,and their correlations with Psoriasis Activity and Severity Index(PASI)score and clinical course of psoriasis vulgaris were invested as well.Results The expressions of PD-1 on CD4^+T cells in patients with psoriasis vulgaris were significantly lower than that in health controls[(14.33±5.16)% vs.(21.63±4.70)%,P〈0.05)].Furthermore,the expressions of PD-1 on CD4^+T cells were negatively correlated with PASI score in these patients(r=0.52,P=0.003),but it did not correlate with clinical course of psoriasis vulgaris(r=0.09,P=0.650).Conclusion The expressions of PD-1 on CD4^+T cells were decreased in patients with psoriasis vulgaris,and correlated with disease severity.
