舒芬太尼不同给药途经术后患者自控镇痛的效果

摘　　要：目的探讨舒芬太尼不同给药途经术后患者自控镇痛(PCA)的镇痛效果。方法食管癌根治手术患者60例均分为两组,术后分别采用静脉舒芬太尼PCA(A组)和皮下注射舒芬太尼PCA(B组)。镇痛药物配方:舒芬太尼7μg/kg+雷莫司琼0.6mg+生理盐水至150ml。首次量2.5ml,背景输注量1.6ml/h,PCA剂量1.6ml,锁定时间10min。术后镇痛72h。评估镇痛期间的VAS疼痛评分,记录PCA按压次数和舒芬太尼用量,观察相关不良反应。结果两组患者术后VAS疼痛评分均显示镇痛效果满意(P>0.05);但A组PCA按压次数[(3.93±0.15)次vs.(12.86±0.80)次]和舒芬太尼用量[(324.80±7.76)mg vs.(379.20±6.64)mg]少于B组(P<0.05)。A组和B镇痛相关不良反应主要有恶心、呕吐,分别为3例和2例。结论静脉或皮下注射舒芬太尼PCA均能获得良好的术后镇痛效果;静脉用药镇痛效果优于皮下注射,能减少镇痛药用量。Objective To investigate the efficacy of patient-controlled analgesia（PCA）with sufentanil in different ways of administration.Methods Sixty patients underwent radical operatin for esophageal cancer were assigned into two groups of A（intravenous PCA with sufentanil,30cases）and B（subcutanious PCA with sufentanil,30cases）.Analgesic solution consisted of sufentanil 7μg/kg plus ramosetron 0.6mg and normal saline 150 ml.Background infusion of 1.6ml/h was used after a bolus dose of 2.5ml was injected.The PCA dose of 1.6ml and lockout interval of 10 minutes were set up.The VAS pain scores were evaluated and PCA numbers and sufentanil consumption were recorded during analgesia for 72 hours.The analgesia-related adverse effects were observed as well.Results Analgesia was satisfectory showed by lower VAS scores in both groups.The PCA number and sufentanil consumption were less in group A than those in group B[（3.93±0.15）times vs.（12.86±0.80）times and（324.80±7.76）mg vs.（379.20±6.64）mg]（P〈0.05）.The main analgesia-related adverse effects were nausea and vomiting,which ocurred in 3cases of group A and 2cases of group B.Conclusion A satisfectory analgesia can be obtained by either intravenous or subcutanious PCA with sufentanil.Analgesic effect of sufentanil PCA is better by intravenous administration than that by subcutanious administration.
