累积高血压暴露与臂踝动脉脉搏波传导速度的关系

摘　　要：目的探讨累积高血压暴露与臂踝动脉脉搏波传导速度（baPWV）的关系。方法在参加开滦集团2006-2007年度健康体检的职工中按抽样标准分层随机抽取观察对象,符合入选标准者5440人,于2012-2013年对该人群进行了第4次健康体检,并检测了该人群的baPWV,最终有3676名观察对象纳入统计分析。累积血压暴露值（cumBP）包括累积收缩压暴露值（cumSBP）和累积舒张压暴露值（cumDBP）,采用逐步线性回归和多因素Logistic回归方法,分析cumSBP和cumDBP与baPWV的关系及累积高血压暴露对baPWV的影响。结果在观察对象3676人中,年龄（55.7±9.8）岁,其中男性2012人,占54.7%,女性1664人,占45.3%。随着累积高血压暴露时间的增加,baPWV和baPWV≥14m/s的检出率呈上升趋势（均P〈0.05）。在逐步线性回归分析中,校正其他混杂因素后,结果发现,cumSBP和cumDBP与baPWV仍呈正线性相关,β值分别为0.20和0.14。在多因素Logistic回归分析中,校正其他混杂因素后,结果发现,累积高血压暴露2年组、4年组、6年组、8年组与暴露0年组比较,均是发生baPWV≥14m/s的危险因素,OR值（95% CI）分别为1.77（1.39~2.26）、2.05（1.53~2.75）、3.20（2.22~4.61）、3.05（1.96~4.74）。结论 cumSBP和cumDBP与baPWV存在正线性相关,累积高血压暴露是baPWV升高的危险因素。Objective To investigate the correlation between cumulative high blood pressure and brachial-ankle pulse wave velocity（baPWV）. Methods A total of 5440 participants in accordance with the standard stratified random sampling were selected from Kailuan Group. Finally,3676 participants with integrated data were recruited into the study. Multivariate linear and Logistic regression models were used to analyze the influence of cumulative blood pressure（cumSBP and cumDBP）and cumulative high blood pressure on baPWV. Results Among 3676 participants[（55.7±9.8）years],2012 individuals were male（54.7%）and 1664 were female（45.3%）. BaPWV and incidence of baPWV≥14 m/s were increased with the expose time of cumulative high blood pressure. CumSBP and cumDBP had significant positive linear relationship with the baPWV（β=0.20,0.14）. Logistic regression analysis showed that higher cumulative high blood pressure was still risk factor of increased baPWV after adjustment for other risk factors,OR（95% CI）were 1.77（1.39-2.26）,2.05（1.53-2.75）,3.20（2.22-4.61）,3.05（1.96-4.74）,respectively. Conclusion CumSBP and cumDBP had significant positive linear relationship with the baPWV,higher cumulative high blood pressure was associated with increased baPWV.
