甲型H1N1流感危重症9例死亡病例回顾性分析

摘　　要：目的:探讨甲型H1N1流感危重症死亡病例的临床特征及死亡原因。方法:2009年11-12月本院甲型H1N1流感病房收治的23例已确诊危重症病例,其中9例死亡,对其临床特点、死亡原因进行综合分析。结果:近80%的死亡病人发病时体温高于38.5℃,病人均有咳嗽和呼吸困难,咯血占一半以上;病人均有干湿啰音;影像弥散实变影(白肺)接近80%;生化检查提示肝,肾,心肌酶均升高。结论:死亡病例的症状、体征均较重,且发展迅速,迅速进入呼吸衰竭。其中2/3病人出现多脏器衰竭。Objective： To investigate clinical characteristics and causes of death in deaths with A H1N1 inlfuenza in critically ill. Methods： From November to December 2009, 23 cases of A H1N1 inlfuenza in our hospital were diagnosed as critically ill patients; and clinical characteristics and causes of 9 cases of death were investigated. Results： There are nearly 80% of deaths patient’s temperature above 38.50℃ during onset period. All patients were cough, breathing difficulties, wet and dry rales. More than half patients were hemoptysis; and closed to 80% patients have white lung. Biochemical examinations, such as liver, kidney, cardiac enzymes were elevated. Conclusion： Symptoms and signs of deaths were severe, showed rapid development into respiratory failure; and two-thirds of the patients showed multiple organ failure.
