中医药治疗糖皮质激素副作用研究进展

摘　　要：中医药治疗GC副作用无论是理论研究还是临床应用,都已经有了广泛的认识。但是我们也发现以下几个问题:1GC导致种种副作用的中医病机认识和中医药治疗大多从肾着手,虽然也有从郁证、少阳郁滞证等来论述,但中医药最大的优势是辨证论治和整体观念的结合,从中医五脏、六腑等角度来讲,有着怎样的影响,还缺乏深入探讨;GC应用过程中中医证候的演变规律,尚缺乏统一的认识;2复方治疗GC副作用,以滋阴补肾、补肾壮阳等补肾类方剂居多且大多以临床观察为主;相关的实验研究较少,药效物质基础、药理作用机制等基础研究和开发还比较薄弱。中药复方是多组分、多环节、多靶点的综合作用,这就要求我们要以中医基础理论为指导,功能主治为依据,药理效应及效应成分为核心,通过系统的实验研究,阐明中药配伍、药效物质基础及效应变化之间的内在联系;3单药研究较少。单药有效成分及作用机制的研究是中药现代化的必经之路,是复方研究的基石和可靠路径之一,对中医药治疗GC副作用的研究亦是如此。分子生物学和现代实验技术的发展,为深入研究中医药治疗GC副作用的研究提供了广阔的空间和平台,进一步研究中药复方和单药治疗GC副作用的具体机制和靶点,能起到明确机理、简化用药的作用,能更好的为临床应用提供理论依据。There is extensive recognition of the side effects of glucocorticoid（ GC） treated with Chinese medicine both in theoretical research and clinical application. However,some problems still exist： a）. Most of the TCM pathogenesis recognition and Chinese medicine treatment for the side effects of GC start from the kidney. Despite the fact that there are some descriptions from the depression syndrome and Shaoyang depression syndrome,the biggest advantage of Chinese medicine lies in the combination of syndrome differentiation and treatment with the concept of holism,for which reason deep exploration on the influence from perspective of the five zang organs and six fu organs lacks and so does the united knowledge of the evolvement rule of the TCM symptoms and signs during the use of GC; b）. Most of the compound formulas for treating the side effects of GC are the key- tonifying ones with effects of nourishing and invigorating the kidney and strengthening yang and it is mostly based on the clinical observation,with few relevant lab researches and weak basic study and development of the pharmacodynamic material basis and pharmacological action mechanism. Chinese medicine formulas have comprehensive multi- component,multi- level and multi- target effects,which request us to take the basic theory of TCM as the guidance,functions and indications as the evidences,pharmacological effects and effective components as the core to expound the drug matching and pharmacodynamic material basis and inner link of the effect changes; c）. Few studies on single drug. The studies on the effective components and action mechanism of the single drug is the only way must be passed in TCM modernization,and is the footstone and reliable route for the compound formula research as well as the research on the side effects of GC treated with Chinese medicine. The development of molecular biology and modern laboratory techniques provide wide space and platform for the research on the side effects of GC treated with Chinese medicine. Furth
