摘 要:目的探讨北京地区环境空气污染物浓度与每日荨麻疹门诊量的关系。方法收集2012年4月1日-2014年4月30日于空军总医院皮肤病医院就诊并被诊断为荨麻疹的北京地区居住的患者的临床资料,同时统计污染物的数据,包括同期PM2.5、PM10、NO_2、SO_2的浓度与气象资料。采用广义相加模型来探讨环境空气污染物与荨麻疹门诊量的关系,以每升高1μg/m3污染物引起的荨麻疹就诊量增加的RR值表示污染对荨麻疹的效应。结果在单污染物模型下,PM2.5、PM10、NO_2和SO_2每增加1μg/m3,分别致当天荨麻疹门诊人次增加的RR为1.000 1(95%CI 0.999 7,1.000 4)、1.000 2(95%CI 0.999 9,1.000 4)、1.002 0(95%CI1.002 8,1.001 1)和1.001 1(95%CI1.000 2,1.002 1)。多污染物模型,分别纳入SO_2,PM10,PM2.5时,NO_2与每日荨麻疹门诊人次的关联强度增强;分别纳入PM2.5,PM10时,SO_2与每日荨麻疹门诊人次的关联强度增强。结论北京地区NO_2、SO_2浓度与荨麻疹门诊人次呈正相关,短期内颗粒物对荨麻疹门诊人次影响不明显。Objective To investigate the short-term association between ambient air pollutants exposure and daily dermatological department visits for urticaria in Beijing, China. Methods Subjects who visited the dermatological department of the General Hospital of Air Force for urticaria from2012.4, 1to 2014.4. 30 were included in the study, date regarding ambient air pollution , the daily 24h average concentration of nitrogen dioxide ( NO2 ) , sulfur dioxide ( SO2 ) and meteorological data ( daily average temperature , relative humidity) were acquired at the same time. We used the generalized additive models (GAM) to analyze the number of dermatology outpatient for urticaria and ambient air pollution, The risk was expressed as a RR and its corresponding 95% confidence interval (95 % CI) byl 1μg/m3 augment of each pollutant Results In a single-air pollutant model , RR and 95% CI by every 1μg/m3 increase in PM2. 5, PM10, NO2 and SO2 were 1. 000 1 (95 % CI 0. 999 7,1. 000 4), 1. 000 2 (95 % CI O. 999 9,1. 000 4), 1. 002 0 (95 % CI1. 002 8, 1. 001 1 ) andl. 001 1 (95% C11. 000 2, 1. 002 1 ) respectively, when muhi-pollutant models included in SO2, PM10, PM2. 5, the correlation of NO2 and daily outpatient urticaria was enhancement; when the models included PM2. 5, PM10 respectively, the strength of association S02 and daily outpatient visits for urticaria enhanced. Conclusion SO2 and NO2are positively associated with daily dermatological department visits for urticaria in Beijing;PMs have little effects on daily outpatient visits for urticaria in a short time.
