腹腔镜全子宫切除术后感染的相关因素分析

摘　　要：目的对腹腔镜全子宫切除术后感染情况及相关因素进行分析。方法将2014年2月至2015年2月于重庆医科大学附属第一医院妇科行腹腔镜全子宫切除患者的临床资料进行回顾性分析，共收集腹腔镜全子宫切除231例，对患者年龄、子宫大小、手术时间、术中出血量、病种、术前住院天数、既往盆腔手术史、引流管等相关因素进行分析。结果231例行腹腔镜全子宫切除患者有23例发生感染，感染率为9．96％。单因素分析结果显示，子宫大小、手术时间、术中出血量、术前住院天数、既往盆腔手术史对术后感染影响较大，差异有统计学意义（P〈O．05）。Logistic多因素回归分析发现子宫大小（OR=3．339，P=0．027）、术前住院天数（OR=1．290，P=O．000）是术后感染发生的独立危险因素。结论腹腔镜全子宫切除术后感染是多因素影响的结果，其发生率高，应根据相应情况，采取有效措施，预防和降低术后感染的发生。Objective To explore and analyze the related factors of nosocomial infection in patients after total laparoscopic hysterectomy. Methods A total of 231 patients undergoing total laparoscopic hysterectomy were enrolled in this study and their data were retrospectively analyzed. The related factors such as uterus size, operation duration, intraoperative blood loss, type of disease, drainage tube, hospital duration before surgery, and history of pelvic surgery were analyzed. Results Among the 231 cases undergoing total laparoscopic hysterecto- my, 23 cases had infection and the infection rate was 9.96%. Univariate analysis showed uterus size, operation duration, intraoperative blood loss, hospital duration before surgery, and history of pelvic surgery had influence on postoperative infection and the difference had statistical significance （P〈0.05）, while the difference of age and drainage tube had no statistical significance on postoperative infection （P〉0.05）. Conclusion The incidence of infection after laparoscopic hysterectomy is related to uterus size, operation duration, intaoperative blood loss, hospital duration before surgery, and history of pelvic surgery.
