摘　　要：目的分析社区服务中心对老年慢性病患者管理及综合干预效果。方法采用自身前后对照方法,选取某社区145例老年慢性病患者进行社区综合干预,并分析干预的效果。结果干预前后老年慢性病患者,在慢性病相关知识的掌握程度、血压及血糖变化、血压及血糖控制率的变化和饮食结构的变化方面,均存在统计学差异(P<0.05);除被动吸烟和控制体重外,吸烟、饮酒、运动锻炼等行为的变化方面存在统计学差异(P<0.05)。结论社区慢性病管理及综合干预能帮助患者建立健康生活方式,实现有效的自我监督,更好地控制自身疾病,减少其并发症的发生,是慢性病防治的有效途径。Objective The study analyzed the effectiveness of management and comprehensive intervention of community service centers on eldedy patients with chronic diseases. Methods A total of 145 elderly patients with chronic diseases were selected from a community for comprehensive intervention, and effectiveness of the intervention was analyzed. Results The elderly patients had significant changes in the understanding of chronic diseases related knowledge, levels of blood pressure and blood sugar, control rate of blood pressure and blood sugar, and dietary structures after the intervention （P〈0.05）. Except for passive smoking and weight control, the changes in smoking, drinking, and exercise were also statistically different （P〈 0.05）. Conclusion The community management and comprehensive intervention of chronic disease is useful in helping the elderly patients build a healthy lifestyle, implement effective self-supervision, better control their diseases, and reduce the incidence of complications, and is therefore an effective route for the prevention and treatment of chronic diseases.
