摘 要:目的 通过对健康人群麻疹抗体水平监测,掌握罗平县麻疹疫苗接种情况和健康人群麻疹免疫抗体水平状况,及时发现高危人群,为控制和消除全县麻疹工作提供科学依据。方法 按照《云南省麻疹等疫苗针对疾病人群抗体水平监测实施方案》,采取分层整群抽样的方法,对全县12个乡镇采集8个年龄组健康人群血清标本,同时采用酶联免疫吸附实验(ELISA)捕捉法测定麻疹IgG抗体。结果 2012年-2014年共检测健康人群血清1 187人份,麻疹IgG抗体阳性1 146人份,阳性率96.55%(1 146/1 187);麻疹IgG抗体保护率达89.22%(1 059/1 187)。2012年-2014年抗体阳性率分别为95.34%,99.22%,95.19%;抗体保护率为86.79%,96.36%,84.86%。不同年龄组间抗体阳性率的比较有显著性差异(χ^2=49.30,P〈0.005);不同性别抗体水平差异无统计学意义(χ^2=0.13,P〉0.05);有、无麻疹疫苗免疫史人群麻疹Ig G抗体阳性率之间差异无统计学意义(χ^2=0.55,P〉0.05)。结论 罗平县从总体上形成了麻疹群体免疫屏障,应确保适龄儿童麻疹疫苗常规免疫2剂接种率达到95%以上,并有计划地对外来人口进行免疫接种,通过各种监测适实开展强化免疫消除易感人群。Objective The aim of this study was to perform healthy crowd measles antibody level monitoring, grasp the Luoping measles vaccination and health crowd measles antibody level status, timely find the high risk population, in order to provide a scientific basis for measles control and eliminate the county work. Methods In the light of the Yunnan province vaccines against diseases such as measles antibody levels crowd monitoring implementation plan, stratified cluster sampling method was adopted. In 12 villages and towns of the county, eight age group healthy crowd serum samples were collected, at the same time enzyme-linked immunosorbent (ELISA) experiment capture method was used to analyze measles IgG antibody. Results From 2012 to 2014, there were a total of 1187 doses healthy crowd serum, 1146 positive doses of measles IgG antibody, with a positive rate of 96.55% (1146/1187). Measles IgG antibody protection ratio was 89.22% (1059/1187). From 2012 to 2014, antibody positive rate were 95.34%, 99.22%, and 95.19%, respectively. Antibody of protection was 86.79%, 96.36%, and 84.86%, respectively. Positive rate of antibody for different age groups had significant difference ( x^2=49.30, P〈 0.005). There was no statistically significant difference for gender antibody levels ( x^2=0.13, P〉 0.13). For groups with or without history of measles vaccine immunization, there was no statistically significant difference for IgG antibody positive rate of measles ( x^2=0.55, P〉 0.55). Conclusion Luoping in general formed the measles immunity barrier. It is suggested to ensure that school-age children take measles routine immunization agent 2 with a coverage of more than 95%, and perform immunization for floating population in a planned way, through a variety of monitoring firm conducting intensive immune elimination of vulnerable groups.
